Er:YAG激光活化荡洗与超声荡洗在磨牙根管治疗中的临床疗效观察

摘要
目的:观察与比较不同的荡洗方法对磨牙根管荡洗的短期与长期的治疗效果,提出适合磨牙根管的治疗方案.方法:选择2019年6月至2020年9月在牙体牙髓科因慢性根尖周炎进行磨牙根管治疗的初诊患者60例,随机分成3组,每组20例.LAI组采用2.5%次氯酸钠溶液+Er:YAG激光髓腔内活化荡洗;UAI组采用2.5%次氯酸钠溶液+超声荡洗;手工组采用2.5%次氯酸钠溶液注射器荡洗.各组根管荡洗前进行采样,荡洗后即刻再次采样.Real-Time PCR法计算比较3组间卟啉单胞菌DNA复制数量,于术后1周、1月、1年进行随访,收集疼痛程度等临床资料和根尖区牙槽骨密度变化等影像资料,进行疗效对比.结果:3组荡洗后卟啉单胞菌DNA数量明显少于荡洗前,且LAI组[(56.8±4.8)%]与UAI组[(53.8±3.8)%]的灭菌率明显大于手工组[(40.0±2.5)%].术后1周LAI组与UAI组疼痛指数[(2.1±0.4),(2.2±0.4)]明显小于手工组(2.8±0.4),差异有统计学意义(P<0.05),但术后1月3组差异无统计学意义(P>0.05).术后1年根尖区牙槽骨阴影LAI组[(1.2±0.1)mm2]与UAI组[(1.2±0.1)mm2]小于手工组[(1.4±0.2)mm2],差异均有统计学意义(P<0.05).结论:在磨牙根管治疗中Er:YAG激光活化荡洗与超声活化荡洗的长短期疗效均优于手工注射器荡洗,但Er:YAG激光活化荡洗与超声活化荡洗之间疗效差异不显著.
Abstract
Objective:To observe and compare the short-term and long-term therapeutic effects of different irrigation methods on root canal treatment of molars,with the goal of proposing an optimal treatment approach to molar root canals.Methods:A total of 60 initial patients undergoing molar root canal treatment due to chronic apical periodontitis in the department of conservative dentistry and endodontics from June 2019 to September 2020 were selected.They were randomly divided into three groups,with 20 patients in each group.The LAI group received irrigation with 2.5%sodium hypochlorite solution combined with Er:YAG laser activation in the pulp chamber.The UAI group received irrigation with 2.5%sodium hypochlorite solution combined with ultrasonic activation.The manual group received irrigation with a 2.5%sodium hypochlorite solution using a syringe.Samples were taken before irrigation in each group.Immediate post-irrigation samples were collected,and Real-Time PCR was employed to compare the quantity of Porphyromonas endodontalis among the three groups.Follow-up assessments were conducted at 1 week,1 month,and 1 year postoperatively,collecting clinical data on pain comparison and imaging data on changes in periapical bone density for efficacy comparison.Results:The quantity of Porphyromonas endodontalis DNA in each group significantly decreased after irrigation.The sterilization rates in the LAI group(56.8±4.8)%and UAI group(53.8±3.8)%were significantly higher than that in the manual group(40.0±2.5)%.At 1 week postoperatively,the pain index in the LAI group(2.1±0.4)and UAI group(2.2±0.4)was significantly lower than that in the manual group(2.8±0.4),with statistical significance(P<0.05).However,no significant differences were observed among the three groups at 1 month postoperatively(P>0.05).At 1 year postoperatively,the changes in periapical bone density in the LAI group[(1.2±0.1)mm2]and UAI group[(1.2±0.1)mm2]were significantly better than those in the manual group[(1.4±0.2)mm2],with statistical significance(P<0.05).Conclusion:In molar root canal treatment,comparing Er:YAG laser activation irrigation,ultrasonic activation irrigation,and manual syringe irrigation,both Er:YAG laser activation irrigation and ultrasonic activation irrigation showed superior short-term and long-term efficacy compared to manual syringe irrigation.However,there was no significant difference in efficacy between Er:YAG laser activation irrigation and ultrasonic activation irrigation.
