摘要

目的:将多学科协作教学模式(Multi-disciplinary treatment,MDT)应用于胸痛护理培训并评价其效果.方法:将南昌市某三级甲等医院两个不同院区的在职护士分为观察组和对照组,研究对象均来自心血管内科、急诊科或CCU,2组研究对象各32例,共64例.其中观察组进行基于MDT的胸痛护理培训,对照组按常规培训,观察2组培训护士首次医疗接触至首份心电图完成时间、首份心电图完成至首份心电图确诊时间、抽血到肌钙蛋白报告时间、门-球时间、导管室激活时间以及急性胸痛患者在就诊期间意外不良事件发生率.结果:观察组所护理患者的首次医疗接触至首份心电图完成时间、首份心电图完成至首份心电图确诊时间、抽血到肌钙蛋白报告时间、门-球时间、导管室激活时间以及急性胸痛患者在就诊期间意外不良事件发生率均优于对照组(P<0.05).结论:将MDT融入胸痛护理培训,有助于护士熟知并掌握胸痛急救处理,在急救过程中灵活应变,有效缩短胸痛患者救治中的多项重要时间;对提升高危胸痛患者救治效率、提高胸痛护士护理能力,改善胸痛患者预后,降低患者就诊期间意外不良事件发生率有重要意义,是临床可积极应用的培训模式.

Abstract

Objective:To apply multidisciplinary collaborative teaching mode to chest pain nursing training and evaluate its effect.Methods:The on-the-job nurses in two different districts of a tertiary hospital in Nanchang were divided into observation group and control group.The subjects were from cardiovascular medicine department,emergency department or CCU,32 cases in each groupwitha total of 64 cases.The observation group received MDT-based chest pain nursing training,while the control group received routine training.The time from the first medical contact to the completion of the first electrocardiogram,the time from the completion of the first electrocardiogram to the diagnosis of the first electrocar-diogram,the time from blood drawing to troponin report,the door-to-balloon time,the activation time of the catheter room and the incidence of complications in patients with acute chest pain during treatment were observed and analyzed.Results:In the observation group,the time from the first medical contact to the completion of the first electrocardiogram,the time from the completion of the first electrocardiogram to the diagnosis of the first electrocardiogram,the time from the blood drawing to the troponin report,the door-ball time,the catheter room activation time and the incidence of complications during the treatment of patients with acute chest pain were significantly decreased,and they were better than those of the control group(P<0.05).Conclusion:Integrating MDT into chest pain nursing training can help nurses to be familiar with and master the first aid treatment of chest pain,be flexible in the first aid process,and effectively shorten many important time in the treatment of chest pain patients.It is of great significance to improve the treatment efficiency of patients with high-risk chest pain,improve the nursing ability of nursesinchest pain care,improve the prognosis of patients with chest pain,and reduce the incidence of complications during treatment.It is a training mode that can be actively applied in clinical practice.
