摘要
目的:研究末端可弯曲输尿管导引鞘(Negative pressure urethral access sheath,NPUAS)联合软性输尿管镜(Flexible stereoscope)治疗肾结石的临床效果.方法:选取本院2023年1月—10月100例直径≤2 cm的肾结石患者,按使用的输尿管导引鞘不同分为对照组(输尿管常规鞘+软性输尿管镜)和观察组(末端可弯曲输尿管导引鞘+软性输尿管镜),各50例,比较2组手术时间、住院时间、住院费用、炎症指标、结石清除率、术后并发症及术后1 d、3 d、6 d疼痛程度.结果:观察组手术时间、住院时间短于对照组,住院费用少于对照组(P＜0.05);观察组血白细胞总数、C反应蛋白、降钙素原低于对照组(P＜0.05);观察组术后感染、出血、输尿管损伤等并发症发生率低于对照组(P＜0.05);观察组结石清除率高于对照组(P＜0.05);观察组术后1 d、3 d、6 d疼痛视觉模拟(Visual Analog Scale,VAS)评分低于对照组(P＜0.05).结论:运用末端可弯曲输尿管导引鞘治疗肾结石,可提高结石清除率,缩短手术时间,减少并发症.
Abstract
Objective:To investigate the clinical efficacy of negative pressure urethral access sheath(NPUAS)combined with flexible ureteroscopy in the treatment of kidney stones.Methods:From January 2023 to October 2023,100 cases with kidney stones with a diameter of≤2 cm in our hospital were selected and randomly divided into two groups using a random number table method(random number generator):control group(conventional ureteral sheath+flexible ureteroscope treatment)and observation group(flexible ureteral guiding sheath+flexible ureteroscope treatment),with 50 cases in each group.The surgical time,hospitalization time,hospitalization costs,inflammatory indicators,stone clearance rate,postoperative complications,and postoperative pain at 1 d,3 d and 6 d were compared between the two groups.Results:The observation group had shorter surgical time and hospitalization time compared to the control group,with lower hospitalization costs(P＜0.05);The total number of white blood cells,C-reactive protein,and procalcitonin in the observation group were lower than those in the control group(P＜0.05).The incidence of postoperative complications such as infection,bleeding,and ureteral injury in the observation group was lower than that in the control group(P＜0.05).The stone clearance rate of the observation group was higher than that of the control group(P＜0.05).The Visual Analog Scale(VAS)scores at 1 d,3 d and 6 d(after operation)of the observation group were lower than those of the control group at each time period(P＜0.05).Conclusion:The use of terminal flexible ureteral guiding sheath for the treatment of kidney stones can improve stone removal efficiency,shorten surgical time,and reduce complications.
