老年糖尿病患者使用胰岛素治疗对低血糖恐惧感的研究进展

摘要
低血糖恐惧感(Fear of hypoglycemia,FoH)在老年糖尿病(Diabetes melliitus,DM)患者中广泛存在,胰岛素注射是产生FoH的重要因素之一.FoH对疾病的治疗产生消极影响,目前医疗机构对老年DM患者FoH的关注度不高,评估工具的应用率相对较低,干预措施掌握能力较弱.本文结合近年来国内外研究文献,从评估量表的发展及应用、FoH的原因分析以及相应干预措施3个方面进行综述.文献复习结果显示,FoH的评估工具发展相对成熟,在发达国家应用也较为广泛,国内尚缺乏本土开发的评估工具,老年DM患者使用胰岛素产生FoH的原因呈多元化,体现在认知不足、用药习惯、个体差异、生活及医疗环境、医疗设备等诸多方面,干预措施主要以评估、认知行为疗法、数字化血糖监测、中医护理技术为主.临床医护人员可根据患者FoH的具体表现形式,依托现有技术,制定个性化的干预策略,提升患者的心理体验和自我管理,降低患者FoH.
Abstract
Fear of hypoglycemia(FoH)is widespread in elderly patients with diabetes melliitus(DM),and insulin injection is one of the important factors for FoH.FoH has a negative impact on the treatment of the disease.At present,medical institutions do not pay much attention to FoH in elderly DM patients,the application rate of assessment tools is relatively low,and the ability to master intervention measures is weak.Based on the domestic and foreign research literature in recent years,this paper reviews the development and application of assessment scales,the cause analysis of FoH and the corresponding intervention measures.The results of literature review showed that the assessment tools for FoH are relatively mature and widely used in developed countries,but there is still a lack of locally developed assessment tools in China.The reasons for FoH in elderly DM patients using insulin are diversified,which are reflected in many aspects such as lack of cognition,medication habits,individual differences,living and medical environment,and medical equipment.The intervention measures mainly included assessment,cognitive behavior therapy,digital blood glucose monitoring,and traditional Chinese medicine nursing technology.Clinical medical staff can tailor to the specific manifestations of FoH,rely on existing technology,and formulate personalized intervention strategies to improve the psychological experience and self-management of patients and reduce FoH of patients.
