正畸矫治术后上颌前牙区龈乳头美学重建的研究进展
Research progress on aesthetic reconstruction of gingival papilla in maxillary anterior teeth after orthodontic treatment

摘要
龈乳头退缩是指两牙之间的三角形牙龈组织出现逐步萎缩或消失,导致该区域出现空隙或黑三角的现象.其中正畸治疗后所造成的上颌前牙区龈乳头退缩的发病率相对较高,有研究表明这种术后楔状隙龈乳头的缺失导致了前牙美学区重建难度加大以及患者满意度同步降低.本文结合近年来国内外研究文献,综述了正畸术后上颌前牙区龈乳头美学重建的非侵入性和侵入性治疗方法.文献复习结果表明,非侵入性治疗中的含氟纳米树脂和生物活性复合树脂操作简单,但其适应证范围有限;侵入性的非手术治疗方法通过注射胶原蛋白与浓缩血小板混合物、再生药物注射干细胞、透明质酸填充物和浓缩生长因子结合低水平激光操作方便且预后较好;侵入性的手术治疗方法主要通过外科移植来达到效果.提示侵入性治疗方法对于正畸矫治术后上颌前牙区的龈乳头美学重建可能是今后口腔临床研究的发展趋势.
Abstract
Gingival papilla recession refers to the gradual atrophy or disappearance of the triangular gingival tissue between the two teeth,resulting in a void or black triangle in this area.The incidence of gingival papilla recession in the maxillary anterior teeth after orthodontic treatment is relatively high.Studies have shown that the loss of gingival papilla in the wedge-shaped space after orthodontic treatment makes it difficult to reconstruct the esthetic zone of anterior teeth and reduces patient satisfaction.This article reviews the methods of aesthetic reconstruction of gingival papilla in the maxillary anterior region after orthodontic treatment by non-invasive and invasive methods based on domestic and foreign research literature in recent years.The literature review showed that non-invasive nano-fluorine resin and bioactive composite resin are simple to operate but have limited indications.The invasive non-surgical method by injecting a mixture of collagen and platelet concentrate,regenerative drug injection of stem cells,hyaluronic acid filler and concentrated growth factor combined with low-level laser is easy to operate and has a good prognosis.Invasive surgical methods are mainly achieved by surgical transplantation.The results of these related methods indicate that invasive treatment methods for aesthetic reconstruction of the gingival papilla in the maxillary anterior region after orthodontic treatment may be the trend of oral clinical research in the future.
