摘要
目的 降低阴囊Paget's病的误诊率,提高早期诊断率及生存率.方法 回顾性研究中山大学附属肿瘤医院(1999 ～2013年)36例阴囊Paget's病患者的临床资料,并进行随访及分析.结果 Ray分期A1期21例;A2期8例;B期6例;D期1例;其中阴囊Paget's病合并汗腺癌6例;早中期患者首选治疗方式为阴囊病灶广泛切除术;D期患者采用放疗.7例术后复发,其中1例行姑息性放疗及泰素+顺铂化疗;2例放疗;4例阴囊Paget's病复发灶切除术.36例患者随访平均时间40个月,3例死于肿瘤转移,生存时间分别为79个月,78个月,23个月;3例死于其他肿瘤,生存时间分别16个月,20个月,17个月.结论 阴囊Paget's病患者早期活检是关键,手术彻底切除病灶是首选治疗方式,早期治疗预后良好.
Abstract
Objectives To reduce the scrotum paget's disease of the misdiagnosis rate,improve the early diagnostic rate and survival rate.Methods A retrospective study of tumor hospital affiliated to sun yat-sen university (1999-2013),the clinical data of 36 cases of paget's disease of the scrotum,and follow-up and analysis.Results Ray stage A1 21 cases;A2 8 cases;B 6 cases; D 1 case;The scrotum Paget's disease combined sweat gland carcinoma 6 cases;The preferred treatment for patients with early metaphase is scrotal lesion wide resection;D period were treated by radiotherapy.7 cases of postoperative recurrence,1 of whom underwent palliative radiotherapy and taxol + cisplatin chemotherapy ;2 cases of radiation ;4 cases of scrotal Paget's disease recurrence resection of oven.36 patients were followed for an average time of 40 months,3 cases died of tumor metastasis,and survival time of 79 month respectively,78 month,23 months;3 cases died of other tumors,survival time 16 months respectively,20months,17 months.Conclusions Scrotal Paget's disease early biopsy is the key,the operation complete resection of lesions is the preferred treatment,early treatment prognosis is good.
