经新型阴囊内窥镜下绿激光附睾囊肿切除术的临床疗效观察

摘要
目的 探讨经新型阴囊内窥镜下绿激光附睾囊肿切除术的安全性和有效性.方法 选取2021年4月至2022年5月在徐州市中心医院收治的35例单侧附睾囊肿患者的临床资料.采用随机数字表法将患者分成经新型阴囊内窥镜下绿激光附睾囊肿切除术组(A组,18例)和开放附睾囊肿切除术组(B组,17例).分析两组患者的手术切口大小、手术时间、术后阴囊疼痛程度、术后阴囊水肿时间以及术后住院天数.结果 两组患者均顺利完成手术.A组患者的手术切口、视觉模拟疼痛评分(VAS)小于B组,差异均有统计学意义(均P＜0.05).A组患者的术后阴囊水肿较B组症状轻,差异有统计学意义(P＜0.05).两组患者术后均未出现阴囊血肿、阴囊感染以及切口愈合延迟等并发症.1个月后复查阴囊彩超,A组患者出现1例轻度睾丸鞘膜积液,随访6个月积液无进展,所有患者均未见附睾囊肿复发.结论 经新型阴囊内窥镜下绿激光附睾囊肿切除术安全有效,具有手术微创、术后疼痛轻以及术后阴囊水肿消退快的特点,适合临床推广应用.
Abstract
Objective To investigate the safety and effectiveness of green laser epididymal cyst resection under a new type of scrotal endoscope.Methods Clinical data of 35 patients with uni-lateral epididymal cysts admitted to the urology department of Xuzhou Central Hospital from April 2021 to May 2022 were selected.All patients were randomly divided into two groups:the new scrotal endo-scopic green laser epididymal cyst resection group(group A,18 cases)and the open epididymal cyst resection group(group B,17 cases).The incision size,operation time,postoperative scrotal pain,postoperative scrotal edema time and postoperative hospital stay were analyzed.Results The operation was successfully completed in both groups.The surgical incision and visual analog pain score(VAS)of group A patients were lower than those of group B,and the differences were statistically significant(all P＜0.05).The postoperative scrotal edema in group A patients was milder than that in group B,and the difference was statistically significant(P＜0.05).There were no postoperative complications such as scrotal hematoma,scrotal infection,or delayed wound healing in both groups of patients.One month later,a follow-up examination of scrotal ultrasound revealed that one patient in group A had mild testicular hydrocele,and there was no progression of hydrocele after a 6-month follow-up.No epididy-mal cyst recurrence was observed in all patients.Conclusions The green laser epididymal cyst resec-tion under a new type of scrotal endoscope through the scrotum is safe and effective,with the character-istics of minimally invasive surgery,mild postoperative pain,and rapid disappearance of postoperative scrotal edema.It is suitable for clinical promotion and application.
