Clinical diagnostic value of serum IP-10 and STAT1 expression levels in urinary tract infection after nephrolithiasis surgery
Objective To investigate the clinical value of serum interferon inducible protein-10(IP-10)and signal transducer and activator of transcription 1(STAT1)expression levels in the di-agnosis of postoperative urinary tract infection in patients with nephrolithiasis.Methods Clinical data of 164 patients with kidney stones who underwent surgery and were hospitalized in Yongzhou Central Hospital and the Second Affiliated Hospital of Guangxi Medical University from March 2019 to Decem-ber 2021 were collected.According to the occurrence of urinary tract infection,the patients were divid-ed into uninfected group(146 cases)and infected group(18 cases).Enzyme-linked immunosorbent assay(ELISA)was performed to measure serum IP-10 and STAT1 levels,automatic microbial identifi-cation instrument was applied to identify and analyze pathogenic bacteria in urine;receiver operating characteristic(ROC)curve was performed to analyze the diagnostic value of serum IP-10 and STAT1 expression levels for postoperative urinary tract infection in patients with nephrolithiasis;multivariate logistic regression analysis was performed to analyze the influencing factors of postoperative urinary tract infection in patients with nephrolithiasis.Results A total of 53 pathogenic bacteria were detected in 18 patients with urinary tract infection,including 18 gram-positive bacteria,33 gram-negative bacte-ria,and 2 fungi.In this study,patients with urinary tract infection were mainly infected with Gram-negative bacteria.The length of hospital stay and 24 h urine volume in the infected group were signifi-cantly higher than those in the uninfected group,and the expression levels of serum IP-10 and STAT1 were higher than those in the uninfected group(all P<0.05).The areas under the curve(AUC)of serum IP-10,STAT1 and their combination in the diagnosis of postoperative urinary tract infection in patients with nephrolithiasis were 0.844,0.785 and 0.893,respectively,the specificities were 81.5%,65.8%and 89.0%,respectively,and the sensitivities were 77.8%,88.9%and 72.2%,respectively.IP-10 and STAT1 were independent risk factors for postoperative urinary tract infection in patients with nephrolithiasis(all P<0.05).Conclusion The detection levels of serum IP-10 and STAT1 can be used as diagnostic indicators for postoperative urinary tract infection in patients with nephrolithiasis.The combination of the two can improve the diagnostic efficiency and provide reference for clinical intervention measures.
