男性精子DNA碎片指数与精子形态及精子运动参数相关性的单中心研究

目的 探讨精子DNA碎片指数(DFI)与精子形态、精子运动参数的相关性.方法 选取2020年5月至2021年7月在本院治疗的475例男性精液样本,根据不同DIF将患者分为DFI<15%组(178例)、15%≤DFI≤30%组(155例)和DFI>30%组(142例),比较三组的精子形态、精子运动参数,采用Pearson相关分析DFI与精液常规指标、精子形态与精子运动参数的相关性.结果 DFI<15%组的前向运动精子百分率(PR)、非前向运动精子率(NP)、PR+NP、前向运动精子浓度、非前向运动精子浓度、前向运动精子总数、非前向运动精子总数大于15%≤DFI≤30%组和DFI>30%组,差异均有统计学意义(均P<0.001);DFI>30%组的不动精子率(IM)、不动精子浓度、不动精子总数大于DFI<15%组和15%≤DFI≤30%组,差异均有统计学意义(均P<0.001).DFI<15%组的正常精子形态率、精子运动曲线速度、精子运动直线速度、路径速度、头部侧摆幅度大于15%≤DFI≤30%组和DFI>30%组,差异均有统计学意义(均P<0.001);DFI>30%组的直线性、前向性、鞭打频率大于DFI<15%组和15%≤DFI≤30%组,差异均有统计学意义(均P<0.001).精子DFI与不动精子率、不动精子浓度、不动精子总数呈正相关,与PR、NP、PR+NP、前向运动精子浓度、非前向运动精子浓度、前向运动精子总数、非前向运动精子总数呈负相关(P<0.05).DFI与正常精子形态率呈负相关(P<0.05);DIF与精子运动直线性、精子运动前向性、鞭打频率呈正相关,与精子运动曲线速度、直线速度、路径速度及头部侧摆幅度呈负相关(均P<0.05).结论 精子DFI与部分精液常规参数、精子正常形态率、精子运动参数存在相关性,可作为精子质量的参考指标.
Correlation between sperm DNA fragmentation index,sperm morphology and sperm motility parameters in single center male semen samples
Objective To investigate the correlation between sperm DNA fragmentation index(DFI)and sperm morphology and sperm motility parameters.Methods Semen samples from 475 male patients treated in our hospital from May 2020 to July 2021 were selected.According to DIF,the patients were divided into 3 groups:DFI<15%group(178 cases),15%≤DFI≤30%group(155 ca-ses)and DFI>30%group(142 cases).Sperm morphology and sperm movement parameters of the three groups were compared.Pearson correlation was used to analyze the correlation between DFI and semen rou-tine indexes,sperm morphology and sperm motility parameters.Results In DFI<15%group,there were statistically significant differences in progressive motility(PR),non-progressive motility(NP),PR+NP,forward motile sperm concentration,non-forward motile sperm concentration,total forward motile sperm,non-forward motile sperm total greater than 15%≤ DFI ≤30%group and DFI>30%group(all P<0.001).The immobile aperm rate(IM),motionless sperm concentration and total motionless sperm in DFI>30%group were higher than those in DFI<15%group and 15%≤DFI≤30%group,and the differences were statistically significant(all P<0.001).There were statistically significant differences in normal sperm morphology rate,curve velocity of sperm movement,linear velocity of sperm movement,path velocity,and lateral head swing amplitude in DFI<15%≤DFI≤30%group and DFI>30%group(all P<0.001).There was no significant difference in motion oscillation among the three groups(P>0.05).The straightness,forward-orientation and whipping frequency of DFI>30%group were higher than those of DFI<15%group and 15%≤DFI≤30%group,and the differences were statistically sig-nificant(all P<0.001).Sperm DFI was positively correlated with the rate of immobile sperm,concen-tration of immobile sperm and total number of immobile sperm,and negatively correlated with PR,NP,PR+NP,concentration of forward motile sperm,concentration of non-forward motile sperm,total num-ber of forward motile sperm and total number of non-forward motile sperm(P<0.05).DFI was nega-tively correlated with normal sperm morphology rate(P<0.05).DIF was positively correlated with sperm straightness,forward orientation and whiplash frequency,and negatively correlated with sperm curve velocity,straight velocity,path velocity and lateral swing amplitude(all P<0.05).Conclusions Sperm DFI is correlated with some semen routine parameters,normal morphology rate of sperm and sperm motility parameters,and can be used as a reference index of sperm quality.
