摘要
目的 探讨白蛋白/球蛋白比率(AGR)预测糖尿病肾病血液透析患者的病死率.方法 选取2010年1月1日至2018年1月1日在本院行规律随访、病情稳定的90例糖尿病肾病血液透析患者,收集患者的基线资料,包括一般资料、实验室检查、合并症、用药情况等,根据血AGR值将患者分为AGR≥1.2组(66例)及AGR＜1.2组(24例),对两组患者的基线资料进行比较分析.结果 两组患者的心血管疾病比例、空腹血糖、糖化血红蛋、白蛋白、总蛋白、AGR、血红蛋白、血肌酐比较,差异均有统计学意义(均P＜0.05).AGR≥ 1.2组和AGR＜1.2组分别有8例(17.8％)和6例(37.5％)患者死亡,单因素和多因素Cox回归分析结果显示,低AGR是糖尿病肾病血液透析患者死亡的独立预测因子.结论 AGR可以预测糖尿病肾病血液透析患者的病死率.
Abstract
Objective To investigate the prediction of mortality in hemodialysis patients with diabetic nephropathy by albumin/globulin ratio(AGR).Methods A total of 90 cases diabetic ne-phropathy patients in hemodialysis who were selected in our hospital and followed up regularly from January 1,2010 to January 1,2018 and whose condition was stable.Baseline data were collected,in-cluding general data,laboratory tests,comorbidities,medication,etc.Patients were divided into AGR≥1.2 group(66 cases)and AGR＜1.2 group(24 cases)according to blood AGR value,and the baseline data of the two groups were compared.Results There were statistically significant differences in cardiovascular disease ratio,fasting blood glucose,glycated red egg,albumin,total protein,globulin ratio,hemoglobin and serum creatinine between the two groups(all P＜0.05).There were 8(17.8％)and 6(37.5％)deaths in AGR≥1.2 group and the AGR＜1.2 group,respectively.Uni-variate and multivariate Cox regression analysis showed that low AGR was an independent predictor of death in hemodialysis patients with diabetic nephropathy.Conclusions The AGR can predict mortality in diabetic nephropathy hemodialysis patients.
