目的 探讨重组人骨形态发生蛋白-2(BMP-2)、重组人骨形态发生蛋白-7(BMP-7)在非肌层浸润性膀胱癌患者癌组织中的表达及临床预后意义.方法 选取2016年1月至2018年1月在本院接受经尿道切除术治疗的118例非肌层浸润性膀胱癌患者为研究对象,采用免疫组化法测定膀胱癌组织(膀胱癌组)及癌旁组织(癌旁组)中BMP-2、BMP-7蛋白的表达水平.采用X2检验分析BMP-2、BMP-7蛋白表达水平与非肌层浸润性膀胱癌患者的临床病理特征关系,采用多因素Cox回归分析探讨影响非肌层浸润性膀胱癌患者预后的危险因素.结果 膀胱癌组的BMP-2、BMP-7蛋白阳性表达率低于癌旁组(均P<0.05).BMP-2、BMP-7蛋白表达与年龄、性别均无明显相关性(均P>0.05),与肿瘤最大径、TNM分期、浸润深度、病理级别、淋巴结转移、复发有明显相关性(均P<0.05),且肿瘤最大径≥5 cm、TNM分期越高、浸润深度越深、病理级别越高、有淋巴结转移、有复发患者的BMP-2、BMP-7阳性表达率较低(均P<0.05).BMP-2、BMP-7阳性的膀胱癌患者的5年生存率明显高于BMP-2、BMP-7的阴性患者[75.0%(30/40)vs.41.0%(32/78)、77.8%(28/36)vs.41.5%(34/82),均 P<0.05].肿瘤最大径≥5 cm、TNM分期Ⅲ~Ⅳ期、有淋巴血管浸润、病理级别高、有复发、BMP-2阴性、BMP-7阴性的膀胱癌患者的5年生存率均显著缩短(均P<0.05).Cox回归分析显示:TNM分期Ⅲ~Ⅳ期、病理级别高、BMP-2阴性、BMP-7阴性是影响膀胱癌患者预后的独立危险因素(均P<0.05).结论 非肌层浸润性膀胱癌患者的癌组织中BMP-2、BMP-7蛋白表达降低,二者与非肌层浸润性膀胱癌的进展程度呈负相关,BMP-2、BMP-7阴性表达是非肌层浸润性膀胱癌预后不良的危险影响因素.
Expression of BMP-2 and BMP-7 in non-muscular invasive bladder cancer patients and their clinical prognostic significance
Objective To investigate the expression and prognostic significance of recombi-nant human bone morphogenetic protein-2(BMP-2)and recombinant human bone morphogenetic pro-tein-7(BMP-7)in patients with non-muscular invasive bladder cancer.Methods A total of 118 pa-tients with non-muscular invasive bladder cancer who underwent transurethral resection in our hospital from January 2016 and January 2018 were selected as the research subjects.The expression levels of BMP-2 and BMP-7 protein in bladder cancer tissue(bladder cancer group)and paracancer tissue(pa-racancer group)were determined by immunohistochemical method.The relationship between BMP-2 and BMP-7 protein expression levels and clinicopathological characteristics of patients with non-muscu-lar invasive bladder cancer were investigated by x2 test.Multivariate Cox regression analysis was used to explore the prognostic risk factors of patients with non-muscular invasive bladder cancer.Results The positive expression rate of BMP-2 and BMP-7 protein in the bladder cancer group was lower than that in the adjacent group(all P<0.05).The protein expression of BMP-2 and BMP-7 was not signif-icantly correlated with age and gender(all P>0.05).It was significantly correlated with tumor maxi-mum diameter,TNM stage,invasion depth,pathological grade,lymph node metastasis and recurrence(allP<0.05).Moreover,the maximum tumor diameter≥5 cm,the higher the TNM stage,the dee-per the invasion depth,the higher the pathological grade,the lower the positive expression rates of BMP-2 and BMP-7 in patients with lymph node metastasis and recurrence(all P<0.05).The 5-year survival rate of BMP-2 and BMP-7 positive patients with bladder cancer was significantly higher than that of BMP-2 and BMP-7 negative patients(75.0%vs.41.0%,77.8%vs.41.5%,all P<0.05).The 5-year survival rate of bladder cancer patients with maximum tumor diameter ≥ 5cm,TNM stage Ⅲ-Ⅳ,lymphatic vascular invasion,high pathological grade,recurrence,BMP-2 negative and BMP-7 negative were significantly shortened(all P<0.05).Cox regression analysis showed that TNM stage Ⅲ to Ⅳ,high pathological grade,negative BMP-2 and negative BMP-7 were independent risk factors for the prognosis of patients with bladder cancer(all P<0.05).Conclusions The ex-pression of BMP-2 and BMP-7 protein in cancer tissues of patients with non-muscle invasive bladder cancer are decreased,and the two are negatively correlated with the progression of non-muscle invasive bladder cancer.Negative expression of BMP-2 and BMP-7 are risk factors for poor prognosis of non-muscle invasive bladder cancer.
