输尿管镜碎石术后残石发生率及其CT预测因素研究

目的 探讨输尿管镜碎石术(URS)后的临床无意义残石(RF)发生率及其CT预测因素.方法 回顾性分析2018年1月至2022年12月本院行连续URS治疗的265例上尿路结石患者的临床资料.根据有无RF将患者分为无RF组(164例)和RF组(101例),比较两组的临床资料,采用单因素和多因素logistic回归分析RF发生的影响因素.结果 单因素logistic回归分析结果显示,结石位置、结石数量、目标结石长径和输尿管镜类型与URS术后发生RF有关(均P<0.05);多因素logistic回归分析结果显示,目标结石长径是URS术后发生RF的影响因素(P<0.05).结论 目标结石长径是URS术后发生RF的影响因素,将CT检测结石长径的阈值定为2 mm能够作为URS疗效评价的有效指标.
The incidence of residual debris after ureteroscopic lithotripsy and its CT predictive factors
Objective To investigate the incidence of clinical nonsensical residual stone(RF)after ureteroscopic lithotripsy(URS)and its CT predictive factors.Methods The clinical data of 265 patients with upper urinary tract stones treated by continuous URS in our hospital from January 2018 to December 2022 were retrospectively analyzed.The patients were divided into non-RF group(164 cases)and RF group(101 cases)according to the presence or absence of RF.The clinical data of the two groups were compared,and the influencing factors of RF occurrence were analyzed by univa-riate and multivariate logistic regression.Results Univariate logistic regression analysis showed that the stone location,number of stones,target stone diameter and ureteroscopy type were associated with RF occurrence after URS(all P<0.05).The results of multivariate logistic regression analysis showed that the length of target stone was an influential factor for RF occurrence after URS(P<0.05).Conclusions Target stone length diameter is an influential factor for the occurrence of RF af-ter URS.The threshold value of CT stone length diameter detection as 2 mm can be used as an effec-tive indicator for the evaluation of URS efficacy.
