目的 探究弥散加权成像(DWI)联合血清Wnt1诱导分泌蛋白1(WISP1)、叉头框转录因子O亚族3a(FOXO3a)对膀胱癌(BC)术前淋巴结(LN)转移的诊断价值.方法 回顾性分析2020年3月至2022年3月本院收治的185例疑似BC患者的临床资料,将其中经病理诊断确诊为BC的104例患者设为BC组,并将其中68例LN转移患者作为LN组,36例无LN转移患者作为无LN组.选择同期在本院体检的98例健康志愿者设为对照组.采用酶联免疫吸附法(ELISA)检测BC患者血清中WISP1、FOXO3a表达水平;采用Pearson法分析BC患者血清中WISP1与FOXO3a的相关性;采用受试者工作特征(ROC)曲线分析DWI联合血清WISP1、FOXO3a对BC术前LN转移的诊断价值.结果 与对照组比较,BC患者血清中的WISP1表达水平显著升高,FOXO3a表达水平显著降低,差异均有统计学意义(均P<0.001);与无LN组比较,LN组患者血清中的WISP1表达水平显著升高,FOXO3a表达水平显著降低(均P<0.001);相关性分析结果显示,BC患者的血清WISP1、FOXO3a表达水平呈负相关(r=-0.346,P<0.001);与无LN组比较,LN组患者的ADC值显著降低(P<0.05);DWI联合血清WISP1、FOXO3a诊断BC术前LN转移的灵敏度高于DWI、血清WISP1、FOXO3a单独诊断(均P<0.05).结论 BC术前LN转移患者血清中WISP1的表达水平显著升高,FOXO3a表达水平显著降低,DWI联合血清WISP1、FOXO3a诊断可以提高BC术前LN转移的诊断效能.
Diagnostic value of DWI combined with serum WISP1 and FOXO3a for preoperative lymph node metastasis of bladder cancer
Objective To explore the diagnostic value of diffusion-weighted imaging(DWI)combined with serum Wnt1 induced secretory protein 1(WISP1)and forkhead box transcription factor O subgroup 3a(FOXO3a)for preoperative lymph node(LN)metastasis of bladder cancer(BC).Methods A retrospective analysis was performed on 185 suspected BC patients admitted to our hospi-tal from March 2020 to March 2022,104 patients diagnosed as BC by pathology were in the BC group,68 patients with LN metastasis were in the LN group,and 36 patients without LN metastasis were in the non LN group.At the same time,98 healthy volunteers in our hospital served as the control group.The expression of WISP1 and FOXO3a in serum was detected by enzyme-linked immunosorbent assay(ELISA).Pearson method was used to analyze the correlation between WISP1 and FOXO3a in serum of BC patients.The ROC curve was drawn to analyze the diagnostic value of DWI combined with serum WISP1 and FOXO3a for preoperative LN metastasis of BC.Results Compared with the control group,the expression level of WISP1 in serum of BC patients was obviously increased,and the expression lev-el of FOXO3a was obviously decreased(all P<0.001).Compared with the non LN group,patients in the LN group showed a significant increase in WISP1 expression levels and a significant decrease in FOXO3a expression levels(all P<0.001).The results of correlation analysis showed that the expres-sion levels of serum WISP1 and FOXO3a in BC patients were negatively correlated(r=-0.346,P<0.001).Compared with the group without LN,the ADC value of patients in the LN group was signifi-cantly reduced(P<0.05).The sensitivity of DWI combined with serum WISP1 and FOXO3a in diag-nosing LN metastasis before BC was higher than that of DWI,serum W1SP1 and FOXO3a alone(P<0.05).Conclusions The expression level of WISP1 in serum of patients with LN metastasis before BC is obviously increased,and the expression level of FOXO3a is obviously decreased.DWI combined with serum WISP1 and FOXO3a can improve the diagnostic efficacy for preoperative LN metastasis of BC.
