冠心病患者疾病不确定感与生活质量的关系—应对方式及抑郁的链式中介作用

The relationship between illness uncertainty and quality of life in patients with coronary heart disease:the chain mediating effect of coping style and depression

目的 探讨应对方式及抑郁在冠心病患者疾病不确定感与生活质量间的链式中介作用.方法 采用疾病不确定感量表、医学应对方式问卷、医院焦虑抑郁量表、生活质量量表对江苏省中医院122 例冠心病患者进行问卷调查.结果 疾病不确定感与抑郁呈正相关(r=0.284,P<0.05),与应对方式、生活质量呈负相关(r=-0.130,P<0.01;r=-0.144,P<0.05).中介作用结果显示,疾病不确定感对生活质量的直接效应显著,应对方式、抑郁在疾病不确定感与生活质量间的独立中介及应对方式-抑郁链式中介作用也显著(Bias-Corrected 95%CI不包含 0).结论 疾病不确定感不仅对冠心病患者生活质量产生直接预测作用,还通过应对方式、抑郁独立中介及应对方式-抑郁链式中介作用对其产生间接预测作用.
Objective To explore the chain mediating effects of coping style and depression in the relationship between illness uncertainty and quality of life in patients with coronary heart disease.Methods A total of 122 coronary heart disease patients from Jiangsu Provincial Hospital of Traditional Chinese Medicine were investigated with Uncertainty in Illness Scale,Medical Coping Modes Questionnaire,Hospital Anxiety and Depression Scale,Quality of Life Scale.Results Illness uncertainty was positively correlated with depression(r=0.284,P<0.05),and negatively correlated with coping style,quality of life(r=-0.130,P<0.01;r=-0.144,P<0.05).Mediating analysis revealed that illness uncertainty played positive roles on quality of life.Coping strategies and depression independently mediated the relationship between illness uncertainty and quality of life,and the chain mediating effect of coping strategies-depression was also significant(Bias-Corrected 95%CI did not contain 0).Conclusions Illness Uncertainty not only directly predicts the quality of life in patients with coronary heart disease,but also would be through the indirect path of coping style,depression,and the chain mediating effect of coping styles-depression.
