基于AHM赋权联合TOPSIS法的参麦注射液临床合理用药评价标准的建立与应用

目的 建立参麦注射液药物利用评价细则,探讨层次分析模型(analytic hierarchy process model,AHM)赋权联合逼近理想值排序(technique for order preference by similarity to solution,TOPSIS)法在参麦注射液药物利用合理性评价中的应用.方法 以参麦注射液说明书为基础,结合相关指南,制定参麦注射液药物利用评价标准;采用AHM与TOPSIS相结合的方法对2022年1月至2022年12月川北医学院附属医院(以下简称"本院")220例使用参麦注射液患者的用药合理性进行回顾性评价.结果 220份评价病历中用药合理病历(Ci≥0.8)6例(2.73%),用药基本合理(0.6≤Ci<0.8)185例(84.09%),用药不合理(Ci<0.6)35例(15.91%).疾病诊断、溶媒选择、给药浓度、特殊人群、不良反应监测与处理、注意事项以及病程记录是导致参麦注射液用药不合理的主要因素.结论 AHM赋权联合TOPSIS法所得评价结果量化、科学可信,可用于参麦注射液药物利用合理性评价.
Establishment and application of evaluation criteria for clinical rational drug use of Shenmai injection based on AHM weighting combined with TOPSIS method
Objective To establish the evaluation rules of drug utilization for Shenmai injection,and to explore the application of analytic hierarchy process model(AHM)weighting combined with technique for order preference by similarity to solution(TOPSIS)method in the evaluation of drug utilization rationality of Shenmai injection.Methods Based on the instructions of Shenmai injection and relevant guidelines,the evaluation criteria for drug utilization of Shenmai injection were established.The rationality of the use of Shenmai injection in 220 patients in the Affiliated Hospital of North Sichuan Medical College(hereinafter referred to as'our hospital')from January 2022 to December 2022 was retrospectively evaluated by AHM combining with TOPSIS.Results There were 6 cases(2.73%)with reasonable medication(Ci≥0.8),185 cases(84.09%)with basically reasonable medication(0.6≤Ci<0.8),and 35 cases(15.91%)with unreasonable medication(Ci<0.6)among the 220 medical records.Disease diagnosis,solvent selection,drug concentration,special population,ADR monitoring and treatment,precautions and course records were the main factors leading to irrational use of Shenmai injection.Conclusion The evaluation results obtained by AHM weighting combined with TOPSIS method are quantitative,scientific and credible,and can be used to evaluate the rationality of drug use of Shenmai injection.
