虚拟现实技术对慢性阻塞性肺疾病患者康复效果的meta分析

目的 通过meta分析评价虚拟现实技术对慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者康复的效果.方法 检索PubMed、Cochrane Library、Web of Science、Embase、中国知网、万方数据库及维普中文期刊全文数据库,收集虚拟现实技术应用于COPD患者康复的临床试验,检索时限为建库至2023年7月.采用RevMan 5.4软件进行数据处理.结果 共纳入15篇文献,包括研究对象1 058例.Meta分析结果显示,虚拟现实康复训练有助于增加COPD患者的 6 分钟步行距离[平均差(mean difference,MD)=21.48,95%置信区间(confidence interval,CI):16.01~26.95,P<0.01],提升COPD患者肺功能,包括血氧饱和度(MD=1.16,95%CI:0.67~1.65,P<0.01)、第1秒用力呼气容积(MD=0.27,95%CI:0.22~0.32,P<0.01)、第 1 秒用力呼气容积与用力肺活量比值(MD=4.51,95%CI:3.34~5.67,P<0.01);改善患者症状控制(MD=-3.05,95%CI:-3.27~-2.83,P<0.01);在改善缺氧程度[均数差(standardized mean difference,SMD)=0.11,95%CI:-0.22~0.45,P=0.51]及生活质量(SMD=-0.28,95%CI:-0.62~0.07,P=0.12)方面,差异无统计学意义.结论 虚拟现实技术可改善COPD患者的运动耐力、肺功能及症状控制程度,应用效果较好.
A meta-analysis of the rehabilitation effect of virtual reality technology on patients with chronic obstructive pulmonary disease
Objective To evaluate the effect of virtual reality technology on the rehabilitation of patients with chronic obstructive pulmonary disease(COPD)by meta-analysis.Methods PubMed,Cochrane Library,Web of Science,Embase,CNKI,Wanfang database and VIP Chinese journal full-text database were searched to collect clinical trials on the application of virtual reality technology to the rehabilitation of COPD patients,and the search time was from the establishment of the database to July 2023.RevMan 5.4 software was used for data processing.Results A total of 15 literatures were included,including 1 058 subjects.The results of meta-analysis showed that virtual reality rehabilitation training could increase the 6-minute walking distance of COPD patients[mean difference(MD)=21.48,95%confidence interval(CI):16.01-26.95,P<0.01],and improve pulmonary function,including blood oxygen saturation(MD=1.16,95%CI:0.67-1.65,P<0.01),forced expiratory volume in the first second(MD=0.27,95%CI:0.22-0.32,P<0.01),and the ratio of forced expiratory volume to forced vital capacity in the first second(MD=4.51,95%CI:3.34-5.67,P<0.01)in COPD patients.Symptom control was improved(MD=-3.05,95%CI:-3.27--2.83,P<0.01).There was no significant difference in improving hypoxia[standardized mean difference(SMD)=0.11,95%CI:-0.22-0.45,P=0.51]and quality of life(SMD=-0.28,95%CI:-0.62-0.07,P=0.12).Conclusion Virtual reality technology can improve the exercise endurance,lung function,symptom control of COPD patients,and the application effect is good.
