自拟祛风养血润肤液药浴联合盐酸左西替利嗪对维持性血液透析患者皮肤瘙痒的疗效

摘要
目的 分析自拟祛风养血润肤液药浴联合盐酸左西替利嗪对维持性血液透析(maintenance hemodialysis,MHD)患者皮肤瘙痒搔抓行为及钙磷代谢的影响.方法 选取2021年1月至2022年8月我院规律MHD且血虚风燥夹瘀型皮肤瘙痒患者80例,随机数字表法分为对照组(常规治疗+盐酸左西替利嗪)及观察组(常规治疗+盐酸左西替利嗪+自拟祛风养血润肤液药浴),每组40例.比较两组治疗前后瘙痒评分、血钙、血磷、甲状旁腺激素(parathyroid hormone,PTH)、超敏C反应蛋白(hypersensitive C-reactive protein,hs-CRP)、白介素(interleukin,IL)-6水平及临床疗效.结果 治疗后,两组瘙痒评分、血磷、PTH、hs-CRP、IL-6水平较前均降低(P＜0.05),且观察组低于对照组(P＜0.05).两组血钙水平较前均升高(P＜0.05),且观察组高于对照组(P＜0.05).观察组疗效明显高于对照组(87.50％vs 67.50％,P＜0.05).结论自拟祛风养血润肤液药浴联合盐酸左西替利嗪对MHD患者皮肤瘙痒疗效较佳,能有效缓解瘙痒,改善钙磷代谢,减轻体内炎症.
Abstract
Objective To analyze the effects of self-designed Qufeng Yangxue Runfu liquid bath combined with levocetirizine hydrochloride on itchy scratching behavior and calcium and phosphorus metabolism in patients with maintenance hemodialysis(MHD).Methods A total of 80 patients with regular MHD and blood vacuity and wind-driness in our hospital from January 2021 to August 2022 were selected and divided into control group(conventional treatment+levocetirizine hydrochloride)and observation group(conventional treatment+levocetirizine hydrochloride+self-designed Qufeng Yangxue Runfu liquid bath)by random number table method,with 40 cases in each group.Pruritus score,blood calcium,blood phosphorus,parathyroid hormone(PTH),hypersensitive C-reactive protein(hs-CRP),interleukin(IL)-6 and clinical efficacy were compared between the two groups before and after treatment.Results After treatment,the levels of pruritus score,blood phosphorus,PTH,hs-CRP and IL-6 in both groups were decreased(P＜0.05),and the observation group was lower than the control group(P＜0.05).The serum calcium level in both groups were higher than before(P＜0.05),and the observation group was higher than the control group(P＜0.05).The curative effect of observation group was significantly higher than that of control group(87.50％vs 67.50％,P＜0.05).Conclusion Self-designed Qufeng Yangxue Runfu liquid bath combined with levocetirizine hydrochloride has a better effect on skin pruritus in MHD patients,which can effectively relieve pruritus,improve calcium and phosphorus metabolism,and reduce inflammation in vivo.
