目的 探究茵栀黄口服液对妊娠期肝内胆汁淤积症(intrahepatic cholestasis of pregnancy,ICP)的疗效.方法 选择2019年6月至2021年6月我院收治的110例ICP患者,根据简单排序随机法将其分为对照组(采用熊去氧胆酸+腺苷蛋氨酸治疗)及研究组(在对照组的基础上给予茵栀黄口服液),每组55例.比较两组患者的临床疗效、瘙痒程度、磷脂转运蛋白(phospholipid transfer protein,PLTP)水平及母婴结局.结果 与对照组相比,研究组总有效率显著增加(P＜0.05).治疗后,两组ICP患者的瘙痒程度评分、PLTP及纤维蛋白原水平均显著降低,凝血酶原时间延长,且研究组改善更明显(P＜0.05).研究组及对照组新生儿不良结局总发生率分别为10.91％和27.27％,差异有统计学意义(P＜0.05).结论 茵栀黄口服液联合腺苷蛋氨酸、熊去氧胆酸治疗ICP患者的效果更佳,能降低患者的瘙痒程度及血清PLTP的表达水平,并能改善凝血功能及新生儿结局.
The effect of Yinzhihuang oral liquid on intrahepatic cholestasis of pregnancy
Objective To explore the curative effect of Yinzhihuang oral liquid on intrahepatic cholestasis of pregnancy(ICP).Methods A total of 110 ICP patients admitted to our hospital from June 2019 to June 2021 were selected and divided into the control group(ursodeoxycholic acid+adenosyl methionine treatment)and the study group(Yinzhihuang oral liquid was given on the basis of the control group)according to simple sequencing random method,with 55 patients in each group.The clinical efficacy,itching degree,phospholipid transfer protein(PLTP)level and maternal and infant outcomes were compared between the two groups.Results Compared with the control group,the total effective rate of the study group was significantly increased(P＜0.05).After treatment,the pruritus score,PLTP and fibrinogen of ICP patients in both groups were significantly reduced,the prothrombin time was increased,and the improvement was more obvious in the study group(P＜0.05).The total incidence of adverse outcomes in the study group and the control group was 10.91％and 27.27％,respectively,with statistical significance(P＜0.05).Conclusion Yinzhihuang oral liquid combined with adenosyl-methionine and ursodeoxycholic acid has a better effect in the treatment of ICP patients,which can reduce the degree of pruritus and the expression level of serum PLTP,and improve the coagulation function and newborn outcome.
