小儿热速清口服液联合头孢噻肟治疗小儿急性上呼吸道感染的临床研究

摘要
目的 探讨小儿热速清口服液联合头孢噻肟治疗小儿急性上呼吸道感染的临床疗效.方法 选取 2020 年 1 月—2022 年 12 月铜陵市妇幼保健院收治的 84 例急性上呼吸道感染患儿,按随机数字表法将所有患儿分为对照组和治疗组,每组各 42 例.对照组静脉滴注注射用头孢噻肟,50 mg/kg加入0.9%氯化钠注射液 100 mL中,2 次/d.治疗组在对照组治疗基础上口服小儿热速清口服液,1≤患儿年龄＜3 岁者 10 mL/次,3≤患儿年龄＜7 岁者 15 mL/次,7≤患儿年龄＜12 岁者 20 mL/次,3 次/d.两组疗程均为4 d.观察两组解热效果及临床症状消失时间,比较治疗前后两组全血淋巴细胞与单核细胞比值(LMR)、中性粒细胞与淋巴细胞比值(NLR)、超敏C反应蛋白(hs-CRP)水平及血清降钙素原(PCT)水平.结果 治疗后,治疗组总有效率为 95.24%,显著高于对照组的 80.95%(P＜0.05).治疗后,治疗组体温恢复时间和解热起效时间均显著短于对照组,治疗组即刻退热率显著高于对照组(P＜0.05).治疗后,治疗组患者鼻塞、流涕、咳嗽、咽痛的消失时间均显著短于对照组(P＜0.05).治疗后,两组全血LMR水平均显著上升,而全血NLR、hs-CRP和血清PCT水平均显著下降(P＜0.05);且治疗后,治疗组全血LMR、NLR、hs-CRP和血清PCT水平改善优于对照组(P＜0.05).结论 小儿热速清口服液联合头孢噻肟治疗小儿急性上呼吸道感染具有较好的临床疗效,能有效缩短患儿症状体征的恢复时间,使机体炎症状态受到抑制,且安全性较佳,值得临床推广应用.
Abstract
Objective To explore the clinical efficacy of Xiaoer Resuqing Oral Liquid combined with cefotaxime in treatment of acute upper respiratory tract infection in children.Methods A total of 84 children with acute upper respiratory tract infection admitted to Tongling Maternal and Child Health Hospital from January 2020 to December 2022 were selected and divided into control group and treatment group according to random number table method,with 42 cases in each group.Patients in the control group were iv administered with Cefotaxime Sodium for injection,50 mg/kg was added into 0.9%sodium chloride injection 100 mL,twice daily.Patients in the treatment group were po administered with Xiaoer Resuqing Oral Liquid on the basis of control group,1 to 3 years old 10 mL/time,3 to 7 years old 15 mL/time,7 to 12 years old 20 mL/time,3 times daily.The treatment course of both groups was 4 d.The antipyretic effect and the disappearance time of clinical symptoms in two groups were observed,a nd the levels of whole blood lymphocyte to monocyte ratio(LMR),neutrophil to lymphocyte ratio(NLR),hypersensitive C-reactive protein(hs-CRP)and serum procalcitonin(PCT)were compared before and after treatment.Results After treatment,the total effective rate of treatment group was 95.24%,which was significantly higher than that of control group(80.95%,P<0.05).After treatment,the temperature recovery time and antipyretic onset time of treatment group were significantly shorter than those of control group,and the immediate antipyretic rate of treatment group was significantly higher than that of control group(P<0.05).After treatment,the disappearance time of nasal congestion,runny nose,cough and sore throat in treatment group was significantly shorter than that in control group(P<0.05).After treatment,whole blood LMR levels were significantly increased in both groups,but whole blood NLR,hs-CRP and serum PCT levels were significantly decreased(P<0.05).After treatment,whole blood LMR,NLR,hs-CRP and serum PCT levels in treatment group were better than those in control group(P<0.05).Conclusion Xiaoer Resuqing Oral Liquid combined with cefotaxime has good clinical efficacy in treatment of acute upper respiratory tract infection in children,and can effectively shorten the recovery time of symptoms and signs in children,inhibit the inflammatory state of the body with good safety,which is worthy of clinical application.
