小儿青翘颗粒联合阿莫西林克拉维酸钾治疗小儿急性化脓性扁桃体炎的临床研究

摘要
目的 探讨小儿青翘颗粒联合阿莫西林克拉维酸钾治疗小儿急性化脓性扁桃体炎的临床疗效.方法 选择2021年8 月—2023 年 8 月在承德市中心医院治疗的 112 例急性化脓性扁桃体炎患儿,依据用药情况分为对照组(56 例)和治疗组(56例).对照组口服阿莫西林克拉维酸钾片,5岁≤年龄＜7岁,半片/次,7岁≤年龄＜12岁,1片/次,2次/d.在对照组的基础上,治疗组口服小儿青翘颗粒,5岁≤年龄＜7岁,7.5 g/次,2 次/d;7岁≤年龄＜12岁,7.5 g/次,3 次/d.两组用药治疗 7 d.观察两组患者临床疗效,比较治疗前后两组患者症状好转时间,匹兹堡睡眠指数量表评分(PSQI),及血清白细胞介素 1β(IL-1β)、降钙素原(PCT)、肿瘤坏死因子-α(TNF-α)和白细胞计数(WBC)水平.结果 治疗后,治疗组总有效率为 96.28%,明显高于对照组(83.92%,P＜0.05).治疗后,治疗组咽痛、发热、扁桃体肿大等症状好转时间均明显早于对照组(P＜0.05).治疗后,两组患儿PSQI评分明显下降(P＜0.05),且治疗组PSQI评分明显低于对照组(P＜0.05).治疗后,两组患者血清IL-1β、WBC、TNF-α、PCT水平明显降低(P＜0.05),且治疗组明显低于对照组(P＜0.05).结论 阿莫西林克拉维酸钾与小儿青翘颗粒协同治疗,能有效改善患儿扁桃体炎症状,明显改善患儿睡眠质量,减弱局部炎性反应.
Abstract
Objective To explore the clinical effect of Xiaoer Qingqiao Granules combined with amoxicillin and clavulanate potassium in treatment of acute suppurative tonsillitis in children.Methods Children(112 cases)with acute suppurative tonsillitis from August 2021 to August 2023 were divided into control(56 cases)and treatment(56 cases)group based on different treatments.Children in the control group were po administered with Amoxicillin and Clavulanate Potassium Tablets,5 years≤age<7 years,half tablet/time,7 years≤age<12 years,1 tablet/time,twice daily.Children in the treatment group were po administered with Xiaoer Qingqiao Granules on the basis of the control group,5 years≤age<7 years,7.5 g/time,twice daily,7 years≤age<12 years,7.5 g/time,3 times daily.Children in two groups were treated for 7 d.After treatment,the clinical evaluations were evaluated,the symptom relief time,the PSQI scores,the levels of serum IL-1β,WBC,TNF-α and PCT in two groups before and after treatment were compared.Results After treatment,the clinical effective rate of the treatment group was 96.28%,which was significantly higher than that of the control group(83.92%,P<0.05).After treatment,the improvement time of symptoms such as sore throat,fever and enlarged tonsil in the treatment group was significantly earlier than that in the control group(P<0.05).After treatment,the PSQI score in two groups was significantly decreased(P<0.05),and the score in the treatment group was significantly lower than that of the control group(P<0.05).After treatment,the serum levels of IL-1β,WBC,TNF-α and PCT in two groups were significantly decreased(P<0.05),which in the treatment group were significantly lower than that in the control group(P<0.05).Conclusion The synergistic treatment of Amoxicillin and Clavulanate Potassium Tablets and Xiaoer Qingqiao Granules can effectively improve the symptoms of tonsillitis,significantly improve the sleep quality and weaken the local inflammatory reaction.
