Application of self-management-oriented 5A nursing model in patients undergoing hepatectomy for liver cancer
Objective To explore the application of the self-management-oriented 5A nursing model in patients undergoing hepatectomy for liver cancer,in order to provide a reference for nursing intervention in patients undergoing liver cancer re-section.Methods A total of 125 liver cancer patients who underwent hepatectomy at the Yancheng First Hospital Affiliated of Nanjing University Medical College,from October 2021 to October 2023,were selected and divided into a control group(62 cases)and an observation group(63 cases)using the random number table method.Patients in the control group re-ceived the conventional nursing model intervention,while those in the observation group received intervention based on the self-management-oriented 5A nursing model.The self-management ability and quality of life before and after intervention,postoperative recovery,and incidence of complications in the two groups of patients were compared.Results The time to postoperative ventilation,postoperative drainage time,wound healing time,and length of hospital stay of patients in the ob-servation group were all significantly shorter than those in the control group(P<0.05).No statistically significant differ-ences were found in the four subscales(self-concept,self-care responsibility,self-care skills,and health knowledge level)of the Exercise of Self-care Agency(ESCA)and the total score between the two groups before intervention(P>0.05).The four subscales and total scores of the ESCA scale in both groups were increased after intervention,and the observation group was higher than the control group,with statistically significant differences(P<0.05).No statistically significant differ-ences were noted in the eight dimensions(role physical,physical functioning,social functioning,bodily pain,role emotion-al,emotional well-being,energy/vitality,and general health perceptions)of the 36-Item Short-Form Health Survey(SF-36)between the two groups before intervention(P>0.05).The eight subscales of the SF-36 scale in both groups were significantly increased after intervention,and the observation group was significantly higher than the control group(P<0.05).The incidence of postoperative complications in the observation group was 1.59%(1/63),which was lower than that in the control group of 12.90%(8/62),with a statistically significant difference(x2=6.664,P=0.010).Conclusion The intervention of the self-management-oriented 5A nursing model in patients undergoing hepatectomy for liver cancer can effectively promote postoperative recovery,enhance self-management ability,increase quality of life,and reduce the risk of complications,which is worth promoting in clinical practice.
