目的 分析体素内不相干运动(IVIM)成像的参数真性扩散系数(D)、假性扩散系数(D*)及灌注分数(f)对肾脏良恶性肿瘤诊断的准确性,以期为临床相关干预提供参考.方法 计算机系统检索Web of Science、PubMed等英文数据库及CNKI、维普、万方等中文数据库中关于IVIM各参数诊断肾脏良恶性肿瘤的相关文献,检索截止时间至2024年3月.对纳入文献的相关数据进行汇总并采用Metadisc 1.4以及STATA 15.1软件进行统计学分析.结果 本研究共纳入9篇文献(中文5篇,英文4篇),包含865例肾脏肿瘤病灶.IVIM参数D、D*、f对肾脏良恶性肿瘤诊断的合并敏感度依次为0.85[95%CI(0.73~0.92)]、0.83[95%CI(0.75~0.88)]、0.68[95%CI(0.58~0.77)],合并特异度依次为 0.92[95%CI(0.84~0.96)]、0.63[95%CI(0.52~0.73)]、0.77[95%CI(0.31~0.96)],综合受试者工作特征(SROC)曲线下面积(AUC)依次为0.95、0.78、0.73.结论 IVIM参数D对区分肾脏良恶性肿瘤具有较高诊断价值,参数D*、f对区分肾脏良恶性肿瘤具有中度诊断价值.
Intravoxel incoherent motion imaging for the diagnostic value of renal benign and malignant tumors:a meta-analysis
Objective To analyze the accuracy of parameters such as true diffusion coefficient(D),pseudo-diffusion coeffi-cient(D*),and perfusion fraction(f)from intravoxel incoherent motion(IVIM)imaging for the diagnosis of benign and malignant renal tumors,aiming to provide support for clinical interventions.Methods A literature search was conducted in English databases including Web of Science,PubMed,and Chinese databases including CNKI,VIP,and Wanfang Data for literature related to the diagnosis of renal benign and malignant tumors using IVIM parameters,up to March 2024.Relevant data from the included literature were summarized and statistically analyzed using Metadisc 1.4 and STATA 15.1 software.Results A total of 9 articles(5 in Chinese and 4 in English)were included in this study,involving 865 renal tumor lesions.The pooled sensitivity of IVIM parameters D,D*,and f for the diagnosis of renal benign and malignant tumors was 0.85[95%CI(0.73-0.92)],0.83[9 5%CI(0.75-0.88)],and 0.68[95%CI(0.58-0.77)],respectively.The pooled spe-cificity was 0.92[95%CI(0.84-0.96)],0.63[95%CI(0.52-0.73)],and 0.77[95%CI(0.31-0.96)],respective-ly.The areas under the summary receiver operating characteristic(SROC)curve were 0.95,0.78,and 0.73,respectively.Conclusion The IVIM parameter D indicates a high diagnostic value for distinguishing between benign and malignant renal tumors,while parameters D*and f demonstrate moderate diagnostic value.
