Correlation of peripheral blood miR-221-3p and miR-124 with ventricular remodeling and myocardial fibrosis in patients with chronic heart failure
Objective To explore the correlation of peripheral blood microRNA-221-Z-3p(miR-221-3p)and miR-124 with ventricular remodeling and myocardial fibrosis in patients with chronic heart failure(CHF).Methods A total of 109 CHF patients admitted to Beijing Luhe Hospital,Capital Medical University from March 2023 to January 2024 were included in the study and divided into classes Ⅱ(n=33),Ⅲ(n=39),and Ⅳ(n=37)according to the New York Heart Association(NYHA)classification.Patient general information was collected,and the expression levels of peripheral blood miR-221-3p and miR-124 in different NYHA classification CHF patients were compared.The ventricular remodeling indicators[left ven-tricular end-diastolic volume(EDV),left ventricular end-systolic volume(ESV),mean wall stress(MWS),and left ven-tricular mass index(LVMI)],and myocardial fibrosis indicators[hyaluronic acid(HA),laminin(LN),type Ⅲ procolla-gen(PCⅢ)]in different NYHA classification CHF patients were detected.The relationship between miR-221-3p,miR-124,and ventricular remodeling and myocardial fibrosis indicators was analyzed.Results The levels of serum total choles-terol(TC),triglycerides(TG),low-density lipoprotein cholesterol(LDL-C),and N-terminal pro-B-type natriuretic peptide(NT-proBNP)in different NYHA classification CHF patients were Ⅱ<Ⅲ<Ⅳ,with significant differences(P<0.05).The levels of serum high-density lipoprotein cholesterol(HDL-C)were Ⅱ>Ⅲ>Ⅳ,with significant differences(P<0.05).The gender composition,age,smoking history,hypertension,diabetes,body mass index(BMI),diastolic pres-sure,and systolic pressure levels in different NYHA classification CHF patients were not statistically significant(P>0.05).The expression levels of peripheral blood miR-221-3p in different NYHA classification CHF patients were Ⅱ<Ⅲ<Ⅳ.with significant differences(P<0.05);the expression levels of miR-124 were Ⅱ>Ⅲ>Ⅳ,with significant differences(P<0.05).The levels of EDV,ESV,MWS,and LVMI in different NYHA classification CHF patients wereⅡ<Ⅲ<Ⅳ,with significant differences(P<0.05);the levels of left ventricular ejection fraction(LVEF)were ⅡⅢ>Ⅳ,with significant differences(P<0.05).The levels of LN,PC Ⅲ,and HA in different NYHA classification CHF patients were Ⅱ<Ⅲ<Ⅳ,with significant differences(P<0.05).Pearson correlation analysis showed the expression level of peripheral blood miR-221-3p in CHF patients was positively correlated with the levels of EDV,ESV,MWS,and LVMI(r=0.387,0.414,0.408,and 0.390,all P<0.05),and negatively correlated with the level of LVEF(r=-0.416,P<0.05).The expression level of peripheral blood miR-124 was negatively correlated with the levels of EDV,ESV,MWS,and LVMI(r=-0.405,-0.432,-0.369,and-0.373,all P<0.05),and positively correlated with the level of LVEF(r=0.439,P<0.05).Pearson correlation analysis indicated the expression level of peripheral blood miR-221-3p in CHF patients was positively correlated with the levels of LN,PC Ⅲ,and HA(r=0.405,0.376,and 0.438,all P<0.05);the expression level of peripheral blood miR-124 was negatively correlated with the levels of LN,PC Ⅲ,and HA(r=-0.464,-0.397,and-0.413,all P<0.05).Conclusion The expression level of peripheral blood miR-221-3p is positively correlated with the progression of ventricular remodeling and myocardial fibrosis in CHF patients,and the expres-sion level of miR-124 is negatively correlated with the progression of ventricular remodeling and myocardial fibrosis.Both can serve as potential biomarkers,providing new insights for clinical assessment of CHF patient disease progression and my-ocardial fibrosis,and aiding in the early identification of high-risk patients and the formulation of individualized treatment strategies.
