Impact of follow-up nursing management model based on structural family therapy on the prognosis of patients undergoing colostomy for rectal cancer
Objective To explore the impact of a follow-up nursing management model based on structural family therapy on the prognosis of patients undergoing colostomy for rectal cancer,aiming to provide a reference for clinical treatment.Meth-ods A total of 102 patients who underwent colostomy for rectal cancer at the First Affiliated Hospital of Air Force Medical University from July 2021 to February 2023 were selected as the study participants.The implementation of the follow-up nursing management model based on structural family therapy in the hospital was used as the cutoff point(February 2022),with 50 patients seen before the implementation(from July 2021 to January 2022)included in the control group,and 52 pa-tients seen after the implementation(from February 2022 to February 2023)in the observation group.The control group re-ceived a conventional follow-up nursing model,while the observation group received a follow-up nursing management model based on structural family therapy.During the 3-month intervention,the incidence of colostomy complications in the two groups was compared;the Exercise of Self-Care Agency(ESCA),the hospital's self-made compliance scale,the Chinese version of the Connor-Davidson Resilience Scale(CD-RISC),the Cancer Fatigue Scale(CFS),and the City of Hope Quality of Life-Ostomy Questionnaire(COH-QOL-OQ)scores were compared before and after the 3-month intervention in both groups.Results The incidence of complications in the observation group was significantly lower than that in the control group(P<0.05).Before intervention,no significant difference was noted in the self-concept,self-care responsibility,self-care skills,and health knowledge level scores of the ESCA between the two groups(P>0.05).After intervention,the scores in these subscales were increased,and the observation group had higher scores than the control group,with signifi-cant differences(P<0.05).The overall treatment compliance rate in the observation group was significantly higher than that in the control group(P<0.05).Before intervention,no significant difference was found in the optimism,resilience,and strength scores of the CD-RISC between the two groups(P>0.05).After intervention,the scores in these subscales showed an increase,and the observation group had higher scores than the control group,with significant differences(P<0.05).Before intervention,no significant difference was observed in the physical fatigue,emotional fatigue,and cognitive fatigue scores of the CFS between the two groups(P>0.05).After intervention,the scores in these subscales were de-creased,and the observation group had lower scores than the control group,with significant differences(P<0.05).Before intervention,no significant difference was found in the physical health,mental health,social health,and spiritual health scores of the COHQOL-OQ between the two groups(P>0.05).After intervention,the scores in these subscales elevated,and the obser-vation group had lower scores than the control group,with significant differences(P<0.05).Conclusion The follow-up nursing management model based on structural family therapy can effectively reduce the risk of postoperative complications,promote psy-chological resilience,alleviate cancer-related fatigue,and positively impact the quality of life and self-care ability of patients.
