家庭医生个性化服务续约与医养结合服务连续性提升研究

作为医养结合制度优势向实际效能转化的重要抓手,家庭医生签约服务整合医疗康复与养老护理,可以为老年人提供兼具综合性与连续性的健康养老服务.实际中,家庭医生签约服务可分为基础服务和个性化服务.相较于基础服务,个性化服务可以更好地满足老年人多元化健康需求,是提升医养结合服务综合性的关键.医养结合服务连续性的提升需要解决个性化服务情景下的家庭医生签约服务续约难的问题.本文采用演化博弈论,解析续约时的家庭医生签约服务与收益,并研究家庭医生与老年人的续约策略选择问题.结果表明:提高家庭医生续约后的收益、降低双方在服务过程中的利益侵占,是解决家庭医生续约难、提升医养结合服务连续性的有效途径.
Research on Renewal of Personalized Family Doctor Services and Continuity Improvement of Integrated Elderly Care and Medical Services
As an important enabler in converting institutional advantages of integrated elderly care and medical services into practical effectiveness,family doctor contract services integrate medical rehabilitation and elderly care to provide comprehensive and continuous elderly care services.In practice,family doctor contracted services can be divided into basic services and personalized services.Compared with basic services,personalized services meet the diversified health needs of the elderly better.Therefore,they are the key to improving the comprehensiveness of integrated elderly care and medical services.However,with the purpose of improving the continuity of integrated elderly care and medical services,it is inevitable to solve the"difficulty in renewal"of family doctor contracted services under the personalized service scenario.The evolutionary game theory was applied to analyze family doctor contracted services and their benefits at the time of contract renewal.The renewal strategy choice of family doctors and the elderly was explored.The results show that increasing the benefits of family doctors'contract renewal and reducing bilateral encroachments are two effective ways of solving the"difficulty in renewal"and improving the continuity of integrated elderly care and medical services.
