Objective To analyze the curative effect of total knee arthroplasty for Ranawat type Ⅱ valgus deformity using me-dial femoral condylar sliding osteotomy.Methods A retrospective analysis was performed on 65 patients with Ranawat type Ⅱvalgus deformity who underwent total knee arthroplasty using sliding osteotomy technique of medial femoral condyle from June 2012 to June 2017.Among them,31 cases underwent intraoperative medial femoral condylar sliding osteotomy(osteotomy group)while 34 cases underwent conventional lateral soft tissue release(conventional group).The incidence of postoperative complications of the two groups were recorded,and the hip,knee and ankle Angle,KSS clinical score of knee joint,KSS func-tional score of knee joint and knee motion were compared between the two groups.Results All patients were followed up for 4.0-7.8 years(mean 6.3 years).Deep venous thrombosis of lower extremity occurred in 6 cases in the conventional group and 4 cases in the osteotomy group,and recovered after anticoagulation and thrombolysis about 2 weeks after surgery.In the osteotomy group,a few screws were removed due to osteoporosis in 3 cases,1 case had a small incision to reinsert the screws due to more screw withdrawal,and the other 2 cases had no obvious symptoms without special treatment.In all patients,the sliding osteoto-my and the femoral condyle healed.Imaging examination showed that the fracture line of the osteotomy disappeared 3-6 months after surgery,with an average of 3.2 months,and no bone nonunion occurred.At the last follow-up,KSS clinical score of knee joint,KSS functional score of knee joint and knee motion in the osteotomy group were better than those in the conventional group(P<0.05),and there was no statistically significant difference in hip,knee and ankle Angle between the two groups(P>0.05).Conclusion The effect of total knee arthroplasty with sliding osteotomy technique of femoral medial condyle in treat-ment of Ranawat type Ⅱ valgus deformity is satisfactory.During the operation,a large release of the lateral soft tissue can be avoided,the operation is relatively simple,and it is easier to achieve the balance of flexion and extension space and internal and lateral soft tissue,and the postoperative stability,force line and function of the knee joint are satisfactory.
