冲击波与中药熏蒸联合治疗职业相关性颈椎病的临床调查

目的 观察冲击波配合中药熏蒸治疗职业相关性颈椎病的疗效.方法 随机数字表法将84例职业相关性颈椎病患者分为对照组(n=42)和观察组(n=42),分别予中药熏蒸和体外冲击波联合中药熏蒸治疗,比较两组临床疗效、疼痛状况、血清疼痛应激因子水平及生活质量.结果 观察组优良率为83.33%,高于对照组的61.90%(x2=4.850,P=0.028).治疗后,两组现时疼痛程度(PPI)评分[(1.26± 0.35)比(1.84±0.42)分,t=6.875]、疼痛视觉量表(VAS)评分[(2.33±0.59)比(4.77±0.81)分,t=15.780],疼痛分级指数(PRI)[(12.25±0.75)比(15.91±0.88)分,t=20.514]、血清神经肱 Y(NPY)[(116.40±10.03)μg/mL 比(128.52±12.46)μg/mL,t=4.911]、神经生长因子(NGF)[(37.35±3.26)pg/mL比(42.41±4.07)pg/mL,t=6.289]、P 物质(SP)[(4.30±0.59)比(5.17±0.78)μg/mL,t=5.765]、SF-36量表生理功能[(58.59±7.57)比(53.14±6.94)分,t=3.493]、生理职能[(62.77±6.83)比(56.95±7.08)分,t=3.834]、活力[(66.35±7.82)比(61.41±6.59)分,t=3.131]、精神健康[(58.02±7.30)比(52.75±6.81)分,t=3.421]、躯体疼痛[(60.80±6.63)比(54.37±6.13)分,t=4.615]及总体健康评分[(61.91±7.39)比(56.15±7.04)分,t=3.657]比较,差异有统计学意义(P<0.05).结论 冲击波配合中药熏蒸治疗职业相关性颈椎病可降低患者疼痛应激因子水平,更有效改善颈肩部疼痛,促进生活质量提高.
Clinical investigation on the shock wave treatment combined with traditional Chinese medicine fumigation for occupation-associated cervical spondylosis
Objective To observe the clinical effect of shock wave combined with traditional Chinese medicine fumigation in the treatment of occupational-associated cervical spondylosis.Methods A total of 84 patients with occupational-associated cervical spondylosis were divided into control group(n=42)and observation group(n=42)by random number table method.The control group was treated with traditional Chinese medicine fumigation,and the observation group was treated with extracorporeal shock wave combined with traditional Chinese medicine fumigation.Clinical effects,pain,serum stress factors and quality of life were compared between the two groups.Results The excellent and good rate in the observation group(83.33%)was significantly higher than that in the control group(61.90%)(x2=4.850,P=0.028).After treatment,the present pain intensity(PPI)score in the control group and the observation group was(1.26±0.35)scores and(1.84±0.42)scores,respectively(t=6.875,P<0.01).The visual analogue scale(VAS)score in the control group and the observation group was(2.33±0.59)scores and(4.77±0.81)scores,respectively(t=15.780,P<0.01).The pain rating index(PRI)score in the control group and the observation group was(12.25±0.75)scores and(15.91±0.88)scores,respectively(t=20.514,P<0.01).The levels of serum neuropeptide Y(NPY)were(116.40±10.03)μg/mL and(128.52±12.46)μg/mL in the observation group and the control group,respectively(t=4.911,P<0.01).The levels of serum nerve growth factor(NGF)were(37.35± 3.26)pg/mL and(42.41±4.07)pg/mL in the observation group and the control group,respectively(t=6.289,P<0.01).The levels of serum substance P(SP)were(4.30±0.59)μg/mL and(5.17±0.78)μg/mL in the observation group and the control group,respectively(t=5.765,P<0.01).The physiological function score was(58.59±7.57)scores and(53.14±6.94)scores in the observation group and the control group,respectively(t=3.439,P<0.01).The physiological function score was(62.77±6.83)scores and(56.95±7.08)scores in the observation group and the control group,respectively(t=3.834,P<0.01).The vitality score was(66.35±7.82)scores and(61.41±6.59)scores in the observation group and the control group,respectively(t=3.131,P<0.01).The mental health score was(58.02± 7.30)scores and(52.75±6.81)scores in the observation group and the control group,respectively(t=3.421,P<0.01).The physical pain score was(60.80±6.63)scores and(54.37±6.13)scores in the observation group and the control group,respectively(t=4.615,P<0.01).The overall health score was(61.91±7.39)scores and(56.15± 7.04)scores in the observation group and the control group,respectively(t=3.657,P<0.01).Conclusion Shock wave combined with traditional Chinese medicine fumigation is markedly effective in the treatment of occupational-associated cervical spondylosis,which can lower the levels of stress factors,more effectively alleviate neck and shoulder pain,and improve quality of life of the patients.
