目的 探讨凉膈白虎汤加减联合低分子肝素(LMWH)治疗重症肺炎的临床疗效,并分析其对中医证候积分、动脉血气分析的影响。方法 选取2021年1月至2023年12月许昌市人民医院呼吸与危重症医学科收治的160例重症肺炎患者纳入研究,按随机数表法分为观察组和对照组各80例。对照组患者给予LMWH治疗,观察组在对照组治疗的基础上给予凉膈白虎汤加减治疗,均连续治疗1周。比较两组患者的临床治疗效果,以及治疗前后的中医证候积分、动脉血气分析参数、呼吸功能和炎症因子水平,同时比较两组患者治疗期间的不良反发生情况。结果 观察组患者的治疗总有效率为93。75%,明显高于对照组的80。00%,差异有统计学意义(P<0。05);观察组患者治疗后的咳嗽、气促、发热、痰壅积分分别为(1。49±0。38)分、(0。85±0。26)分、(1。11±0。52)分、(0。93±0。34)分,明显低于对照组的(2。02±0。45)分、(1。20±0。37)分、(1。74±0。58)分、(1。36±0。39)分,差异均有统计学意义(P<0。05);观察组患者治疗后的动脉氧分压(PaO2)、氧合指数(OI)分别为(96。21±1。04)mmHg、(320。68±25。18)mmHg,明显高于对照组的(94。16±1。25)mmHg、(285。19±21。44)mmHg,动脉二氧化碳分压(PaCO2)为(46。23±4。8 4)mmHg,明显低于对照组的(52。12±5。19)mmHg,差异均有统计学意义(P<0。05);观察组患者治疗后的呼吸频率为(41。13±3。51)次/min,明显低于对照组的(46。28±4。04)次/min,呼气中段流量(MFF)、呼吸峰流速(PEF)分别为(1。52±0。33)L/s、(2。47±0。39)L/s,明显高于对照组的(1。03±0。27)L/s、(1。88±0。32)L/s,差异均有统计学意义(P<0。05);观察组患者治疗后的血清C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白介素-8(IL-8)、高迁移率族蛋白B1(HMGB1)水平分别为(8。13±2。04)mg/L、(24。32±3。60)pg/mL、(31。15±4。81)ng/L、(35。62±8。23)ng/mL,明显低于对照组的(11。29±2。25)mg/L、(28。76±4。05)pg/mL、(37。64±5。22)ng/L、(48。70±9。55)ng/mL,差异均有统计学意义(P<0。05);治疗期间,观察组和对照组患者的不良反应发生率分别为12。50%、10。00%,差异无统计学意义(P>0。05)。结论 凉膈白虎汤加减联合LMWH治疗重症肺炎可缓解患者的临床症状,改善动脉血气、呼吸功能,抑制炎症反应,疗效确切且具有一定安全性。
Efficacy of modified Liangge Baihu Decoction combined with low-molecular-weight heparin in the treatment of severe pneumonia and its impact on arterial blood gas analysis parameters
Objective To investigate the clinical efficacy of modified Liangge Baihu Decoction combined with low-molecular-weight heparin(LMWH)in the treatment of severe pneumonia,and analyze its impact on TCM syndrome score and arterial blood gas analysis.Methods A total of 160 patients with severe pneumonia admitted to the Depart-ment of Respiratory and Critical Care Medicine,Xuchang People's Hospital from January 2021 to December 2023 were selected for inclusion in the study.According to the random number table method,the patients were divided into an ob-servation group and a control group,with 80 cases in each group.Patients in the control group were treated with LM-WH,while those in the observation group were treated with modified Liangge Baihu Decoction on the basis of the treat-ment in the control group.All patients were treated continuously for 1 week.The clinical efficacy of the two groups of patients was compared,as well as the TCM syndrome scores,arterial blood gas analysis parameters,respiratory function,and inflammatory factor levels before and after treatment.The adverse reactions during treatment were also compared be-tween the two groups of patients.Results The total effective rate of the observation group was 93.75%,which was signifi-cantly higher than the control group's 80.00%(P<0.05).The scores of cough,shortness of breath,fever,and phlegm in the observation group after treatment were(1.49±0.38)points,(0.85±0.26)points,(1.11±0.52)points,and(0.93±0.34)points,respectively,which were significantly lower than(2.02±0.45)points,(1.20±0.37)points,(1.74±0.58)points,and(1.36±0.39)points in the control group(P<0.05).The partial pressure of oxygen in arterial blood(PaO2)and oxygenation index(OI)of the patients in the observation group after treatment were(96.21±1.04)mmHg and(320.68±25.18)mmHg,re-spectively,which were significantly higher than(94.16±1.25)mmHg and(285.19±21.44)mmHg of the control group;the PaCO2 was(46.23±4.84)mmHg,which was significantly lower than(52.12±5.19)mmHg of the control group;the differences were statistically significant(P<0.05).The respiratory rate of patients in the observation group after treatment was(41.13±3.51)times/min,which was significantly lower than(46.28±4.04)times/min of the control group;the mid-expiratory flow rate(MFF)and peak expiratory flow rate(PEF)were(1.52±0.33)L/s and(2.47±0.39)L/s,respec-tively,which were significantly higher than(1.03±0.27)L/s and(1.88±0.32)L/s of the control group;the differences were statistically significant(P<0.05).The levels of serum C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-8(IL-8),and high mobility group box-1 protein(HMGB1)in the observation group after treatment were(8.13±2.04)mg/L,(24.32±3.60)pg/mL,(31.15±4.81)ng/L,and(35.62±8.23)ng/mL,respectively,which were signifi-cantly lower than(11.29±2.25)mg/L,(28.76±4.05)pg/mL,(37.64±5.22)ng/L,and(48.70±9.55)ng/mL in the control group(P<0.05).During the treatment period,the incidence of adverse reactions in the observation group and control group was 12.50%and 10.00%,respectively,with no statistically significant difference(P>0.05).Conclusion Modi-fied Liangge Baihu Decoction combined with LMWH in the treatment of severe pneumonia can alleviate the clinical symptoms of patients,improve arterial blood gas and respiratory function,inhibit inflammatory reactions,and has a defi-nite efficacy and certain safety.
