Efficacy of modified Liangge Baihu Decoction combined with low-molecular-weight heparin in the treatment of severe pneumonia and its impact on arterial blood gas analysis parameters
Objective To investigate the clinical efficacy of modified Liangge Baihu Decoction combined with low-molecular-weight heparin(LMWH)in the treatment of severe pneumonia,and analyze its impact on TCM syndrome score and arterial blood gas analysis.Methods A total of 160 patients with severe pneumonia admitted to the Depart-ment of Respiratory and Critical Care Medicine,Xuchang People's Hospital from January 2021 to December 2023 were selected for inclusion in the study.According to the random number table method,the patients were divided into an ob-servation group and a control group,with 80 cases in each group.Patients in the control group were treated with LM-WH,while those in the observation group were treated with modified Liangge Baihu Decoction on the basis of the treat-ment in the control group.All patients were treated continuously for 1 week.The clinical efficacy of the two groups of patients was compared,as well as the TCM syndrome scores,arterial blood gas analysis parameters,respiratory function,and inflammatory factor levels before and after treatment.The adverse reactions during treatment were also compared be-tween the two groups of patients.Results The total effective rate of the observation group was 93.75%,which was signifi-cantly higher than the control group's 80.00%(P<0.05).The scores of cough,shortness of breath,fever,and phlegm in the observation group after treatment were(1.49±0.38)points,(0.85±0.26)points,(1.11±0.52)points,and(0.93±0.34)points,respectively,which were significantly lower than(2.02±0.45)points,(1.20±0.37)points,(1.74±0.58)points,and(1.36±0.39)points in the control group(P<0.05).The partial pressure of oxygen in arterial blood(PaO2)and oxygenation index(OI)of the patients in the observation group after treatment were(96.21±1.04)mmHg and(320.68±25.18)mmHg,re-spectively,which were significantly higher than(94.16±1.25)mmHg and(285.19±21.44)mmHg of the control group;the PaCO2 was(46.23±4.84)mmHg,which was significantly lower than(52.12±5.19)mmHg of the control group;the differences were statistically significant(P<0.05).The respiratory rate of patients in the observation group after treatment was(41.13±3.51)times/min,which was significantly lower than(46.28±4.04)times/min of the control group;the mid-expiratory flow rate(MFF)and peak expiratory flow rate(PEF)were(1.52±0.33)L/s and(2.47±0.39)L/s,respec-tively,which were significantly higher than(1.03±0.27)L/s and(1.88±0.32)L/s of the control group;the differences were statistically significant(P<0.05).The levels of serum C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-8(IL-8),and high mobility group box-1 protein(HMGB1)in the observation group after treatment were(8.13±2.04)mg/L,(24.32±3.60)pg/mL,(31.15±4.81)ng/L,and(35.62±8.23)ng/mL,respectively,which were signifi-cantly lower than(11.29±2.25)mg/L,(28.76±4.05)pg/mL,(37.64±5.22)ng/L,and(48.70±9.55)ng/mL in the control group(P<0.05).During the treatment period,the incidence of adverse reactions in the observation group and control group was 12.50%and 10.00%,respectively,with no statistically significant difference(P>0.05).Conclusion Modi-fied Liangge Baihu Decoction combined with LMWH in the treatment of severe pneumonia can alleviate the clinical symptoms of patients,improve arterial blood gas and respiratory function,inhibit inflammatory reactions,and has a defi-nite efficacy and certain safety.
