Influencing factors of malnutrition in elderly patients with coronary heart disease and construction of Nomogram prediction model
Objective To explore the influencing factors of malnutrition in elderly patients with coronary heart disease and construct a nomogram prediction model.Methods A prospective study was conducted on 352 elderly pa-tients with coronary heart disease admitted to the Second Affiliated Hospital of Zhengzhou University from January 2020 to January 2024.Based on the presence or absence of concurrent malnutrition,the subjects were categorized into the malnutrition group and the non-malnutrition group.The clinical data of the two groups of patients were compared.The influencing factors of malnutrition in elderly patients with coronary heart disease were analyzed by univariate analy-sis,LASSO,and logistic regression.A Nomogram prediction model was established for malnutrition in elderly patients with coronary heart disease.Receiver operating characteristic(ROC)curve,calibration curve,and decision curve analy-sis(DCA)were used to evaluate the performance of the Nomogram prediction model.Results Among the 352 elderly patients with coronary heart disease,82 were malnourished and 270 patients were not malnourished,with the incidence of malnutrition of 23.30%.There were significant differences in age,disease duration,number of complications,C-reac-tive protein(CRP),B-type brain natriuretic peptide(BNP),regular weekly exercise,anxiety and depression,standardized swallowing assessment(SSA)score,health belief and behavior level scores between the two groups of patients(P<0.05).Longer disease duration,number of complications≥2,high C-reactive protein(CRP)levels,anxiety and depres-sion,high SSA scores,regular weekly exercise,high health belief level scores,and high health behavior level scores were all factors that affect malnutrition in elderly patients with coronary heart disease(P<0.05).A Nomogram prediction model for malnutrition in elderly patients with coronary heart disease was established based on the above influencing fac-tors.ROC curve analysis showed that the area under the curve(AUC)of the Nomogram prediction model for predicting malnutrition in elderly patients with coronary heart disease was 0.89,and the overall trend of the model calibration curve was close to the ideal curve,indicating that the model had good fitting.DCA curve suggested that when the risk thresh-old was between 0.28 and 0.83,the model had good clinical net benefits.Conclusion Disease duration,number of com-plications,CRP,regular weekly exercise,anxiety and depression,health belief score,health behavior score,and SSA score are the influencing factors of malnutrition in elderly patients with CHD,and Nomogram prediction model based on the influencing factors has certain clinical value.
