Application of AngioJet mechanical thrombectomy in the treatment of cerebral hemorrhage combined with deep venous thrombosis
Objective To explore the clinical application value of AngioJet mechanical thrombectomy in pa-tients with cerebral hemorrhage complicated by deep venous thrombosis(DVT)of the lower extremities.Methods The clinical and imaging data of 17 patients with cerebral hemorrhage and DVT of the lower extremities treated with the AngioJet thrombectomy device in the Department of Vascular Interventional Surgery,Zhangjiajie People's Hospital from January 2020 to January 2024 were retrospectively analyzed.The thrombus removal and prognosis were ana-lyzed.Results All patients underwent femoral-popliteal vein puncture under DSA guidance,with filter implantation and AngioJet thrombectomy.Postoperative angiography showed that grade Ⅲ thrombus was cleared in 12 cases,grade Ⅱ in 4 cases,and grade Ⅰ in 1 case.The symptoms of lower limb swelling and pain were significantly improved,and the differences in leg circumference of the thigh and calf before and after surgery were statistically significant(P<0.05).The success rate of the surgery was 100%,and no fatal pulmonary embolism occurred during the perioperative period.One patient with primary thrombocythemia developed re-thrombosis at 6 days after treatment.One patient experienced mild renal impairment,which reversed after active treatment.One patient,who had experienced four episodes of re-current cerebral hemorrhage in the past two years,died at home from a recurrent cerebral hemorrhage at 54 days af-ter surgery.After 3 months of follow-up,symptoms resolved in 16 patients,and venous luminal blood flow was re-stored.Conclusion AngioJet mechanical thrombectomy can be an effective and safe treatment method for cerebral hemorrhage complicated by DVT.
