炎调方调控LncRNA-Miat对急性脓毒症患者免疫功能及肠道微生态的保护作用

目的 探讨炎调方调控长链非编码RNA-心肌梗死相关转录本(LncRNA-Miat)对急性脓毒症患者肠道微生态、免疫功能的保护作用.方法 选取 2020 年 1 月至 2022 年 10 月收治的急性脓毒症患者 108 例,根据随机数字表法分为对照组和观察组,每组 54 例.对照组采取常规对症治疗,观察组采用常规对症治疗+炎调方,2 组均连续治疗 10 d.观察组 6 例因意识改变或昏迷无法口服中药退出研究,为保持例数一致,对照组剔除 6 例,观察组和对照组各有 48 例进入研究.统计 2 组中医疗效、中医证候积分、LncRNA-Miat、磷脂酶张力蛋白同源物(PTEN)表达水平、免疫功能[核因子κB(NF-κB)mRNA、核苷酸结合寡聚化结构域样受体蛋白 3(NLRP3)mRNA、白介素-1β(IL-1β)、IL-18]、肠道微生态(乳酸杆菌、肠杆菌、葡萄球菌)、28 d病死率.结果 观察组总有效率为 83.33%(40/48)高于对照组62.50%(30/48),观察组发热、口渴饮冷、汗出、气喘、乏力评分低于对照组,差异均有统计学意义(P<0.05);观察组PTEN、LncRNA-Miat、NF-κB、NLRP3 mRNA、IL-1β、IL-18、葡萄球菌、肠杆菌低于对照组,乳酸杆菌高于对照组(P<0.05);与对照组相比,观察组APACHEⅡ、SOFA较低(P<0.05);2 组28d病死率对比,差异无统计学意义(P>0.05).结论 联合炎调方治疗急性脓毒症患者,可有效调控LncRNA-Miat表达,改善患者肠道微生物平衡,提高机体免疫,缓解临床病症,促进病情好转.
Yantiao Formula protects immune function and intestinal microecology in patients with acute sepsis by regulating lncRNA-Miat
Objective To investigate the protective effects of Yantiao Formula on intestinal microecology and immune function in patients with acute sepsis by regulating long non-coding RNA-myocardial infarction associated transcript(LncRNA-Miat).Methods A total of 108 patients with acute sepsis in our hospital from January 2020 to October 2022 were selected and randomly divided into control group(n=54)and observation group(n=54)according to random number table method.All patients were treated with conventional symptomatic treatment,and those in observation group were additionally given Yantiao Formula.The treatment lasted for 10 days.Six patients in the observation group dropped out due to changes in consciousness or coma.In order to maintain the same number of cases,6 cases were excluded from the control group,and 48 cases in both the observation group and the control group entered the study.The traditional Chinese medicine(TCM)efficacy,TCM syndrome score,LncRNA-Miat,expression levels of phospholipase tensin homology(PTEN),immune function factors(nuclear factor κB[NF-κB],nucleotide binding oligomeric domain like receptor protein 3[NLRP3],interleukin-1β[IL-1β],IL-18),intestinal microecology(Lactobacillus,Enterobacter,Staphylococcus),and 28-day mortality were compared.Results After treatment,the total effective rate of the observation group was significantly higher than that of control group(83.33%[40/48)vs 62.50%[30/48]),and the scores of fever,thirst and cold,sweating,asthma and fatigue were significantly lower than those of the control group(P<0.05).After treatment,expression levels of PTEN,LncRNA-Miat,NF-κB,NLRP3,IL-1β,and IL-18,and relative abundances of Staphylococcus and Enterobacter were significantly lower in the observation group than those of control group,and the relative abundance of Lactobacillus was significantly higher(P<0.05).After treatment,the Acute Physiology and Chronic Health EvaluationⅡ(APACHEⅡ)and the Sequential Organ Failure Assessment(SOFA)scores in observation group were significantly lower than those of control group(P<0.05).There was no significant difference in the 28-day mortality between the two groups(P>0.05).Conclusion The combination of Yantiao Formula in the treatment of acute sepsis can effectively regulate the expression of LncRNA-Miat,improve the balance of intestinal microorganisms,enhance immunity,alleviate clinical symptoms,and promote the acceptable outcome.
