Influence of shared decision-making on nutritional support at home provided by caregivers on the nutritional status,psychological status and quality of life in patients after esophageal cancer
Objective To study the shared decision-making on nutritional support at home provided by caregivers on the quality of life,nutritional status and psychological status after surgery for esophageal cancer patients.Methods A total of 86 patients with surgically treated esophageal cancer from July 2019 to September 2022 were selected.They were randomly divided into control group and observation group according to the envelope method.Patients in the control group(42 cases)were treated by conventional treatment after surgery,and those in the observation group(44 cases)were treated with conventional treatment plus shared decision-making on nutritional support at home provided by caregivers.Postoperative recovery,complications,sleep quality,nutritional indicators,quality of life,and psychological conditions before and after intervention were compared between groups.Results There were no significant differences in the first time of anal exhaust,time of recovery of bowel sound,and time of first defecation before intervention between groups(P>0.05).Before intervention,hemoglobin(Hb),albumin(ALB),prealbumin(PA),transferrin(TRF),the Self-rating Depression scale(SDS)score,the Pittsburgh sleep quality index(PSQI),the Self-rating Anxiety Scale(SAS)score,role function score,physical function score,social function score,and emotional function score were comparable between groups(P>0.05).The time of first eating in the observation group was significantly shorter than that of the control group(P<0.05).After intervention,PA,ALB,Hb,TRF,role function score,emotional function score,physical function score and social function score were significantly higher than those before intervention,which were significantly higher in the observation group than those of the control group(P<0.05).After intervention,the SAS score,SDS score and PSQI were significantly lower than those before intervention,which were significantly lower in the observation group than those of the control group(P<0.05).The overall incidence of complications was significantly lower in the observation group than that of the control group(P<0.05).Conclusion Shared decision-making on nutritional support at home provided by caregivers significantly improves nutritional status,psychological status and life quality in esophageal cancer patients after surgery,which is worth clinical promotion.
