Effect of Poria Licorice mixture assisted Western medicine on the NF-κB signaling pathway in patients with chronic obstructive pulmonary disease complicated with muscle attenuation and its molecular mechanism
Objective To investigate the effect of Poria Licorice mixture assisted Western medicine on the nuclear factor-kappa B(NF-κB)signaling pathway in patients with chronic obstructive pulmonary disease(COPD)complicated with muscle attenuation and its molecular mechanism.Methods A total of 104 COPD patients combined with muscle attenuation in our hospital from August 2020 to August 2022 were selected and randomly divided into control group and observation group,with 52 cases in each group.Patients in the control group were treated with Western medicine,and those in the observation group were treated with Western medicine + Poria Licorice mixture.The traditional Chinese medicine(TCM)efficacy,adverse events,TCM syndrome scores before and after treatment,mRNA and protein levels of NF-κB,relative levels of interleukin-8(IL-8),IL-1 and tumor necrosis factor-α(TNF-α),nutritional indexes(transferrin[Tf],hemoglobin[Hb],albumin[ALB])and muscle attenuation indexes(grip strength test,the Timed Up and GO Test[TUGT],the Five Times Sit to Stand Test[FTSST]and the Strength,Assistance with Walking,Rising from a Chair,Climbing Stairs,and Falls[SARC-F]Questionnaire)were compared between groups.Results The total TCM effective rate was significantly higher in the observation group than that of the control group(86.54%[45/52]vs 67.31%[35/52],P<0.05).After 1 month and 3 months of treatment,TCM syndrome scores in the observation group were significantly lower than those of the control group(P<0.05).The mRNA and protein levels of NF-κB,relative levels of IL-8,IL-1 and TNF-α after 1 month and 3 months of treatment were significantly lower than those of the control group(P<0.05).After 3 months of treatment,patients in the observation group presented significantly higher grip strength,and lower FTSST,TUGT and SARC-F score than those of the control group(P<0.05).During the treatment,there were no obvious adverse events in both groups.Conclusion Poria Licorice mixture assisted Western medicine in the treatment of COPD complicated with muscle attenuation has a definite effect.It can reduce inflammation,improve nutritional status and clinical symptoms by inhibiting the NF-κB signaling pathway,with a high safety.
