Pulsed CO2 lasers combined with basic periodontal therapy for adolescent gingivitis:a prospective split-mouth self-controlled study
Objective To observe the efficacy of pulsed CO2 lasers combined with basic periodontal therapy on adolescent gingivitis via conducting a split-mouth self-controlled study. Methods A total of 200 adolescent gingivitis patients were selected as the control group. The left and right mouth were self-controlled,with one side treated with basic periodontal therapy(control group),and the other side treated with pulsed CO2 lasers combined with basic periodontal therapy(treatment group). The improvement time of gingival swelling and bleeding were compared and analyzed between the two groups. The number of probing depth(PD)>3mm,the number of positive sites of bleeding on probing(BOP),the percentage of BOP,gingival index(GI),bleeding index(BI),plaque index(PLI)and interleukin-1β(IL-1β),C-reactive protein(CRP),prostaglandin E-2(PGE-2),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)levels in gingival crevicular fluid at 7 days and 6 weeks after treatment were compared. Results Gingival swelling and bleeding in the treatment group was significantly improved earlier than those in the control group(both P<0.05).After 7 days of treatment and 6 weeks of treatment, the site number of PD>3mm,the positive site number of BOP and the percentage of BOP were significantly reduced in both groups, which were significantly lower in the treatment group than in control group(P<0.05).After 7 days of treatment and 6 weeks of treatment,PLI,GI and BI in both groups were significantly reduced, which were significantly lower in the treatment group than in the control group(P<0.05).After 6 weeks of treatment,IL-1β,CRP,PGE-2,TNF-α and IL-6 levels in the gingival crevicular fluid were significantly reduced in both groups, which were significantly lower in the treatment group than in the control group(P<0.05).Conclusion Compared with the monotherapy of basic periodontal treatment, its combination with pulsed CO2 lasers has a better clinical efficacy on adolescent gingivitis, which can improve the clinical symptoms of patients faster and reduce local inflammatory response.
