Application of high-resolution vascular wall imaging to enhancement and post-treatment evaluation of responsible plaques in acute stroke patients
Objective To explore the value of high-resolution vascular wall imaging(HR-VWI)in the detection of responsible plaques in acute stroke patients.Methods A total of 85 eligible acute stroke patients admitted from January 2020 to December 2022 were selected as study subjects,all of whom underwent HR-VWI scanning.After excluding 3 cases with incomplete clinical data,a total of 82 patients were divided into the responsible plaque group(vascular stenosis≥30%,42 cases)and non-responsible plaque group(vascular stenosis<30%,40 cases).The number of plaques and imaging features,including the degree of vascular stenosis,degree of plaque enhancement,index of plaque enhancement,incidence of positive remodeling,remodeling index,and plaque load were analyzed and compared between the two groups.Patients in both groups were treated with antiplatelet drugs and intensive lipid-lowering therapy,and followed up by HR-VWI.Results A total of 84 plaques were detected in 82 acute stroke patients,including 44 responsible plaques and 40 non-responsible plaques.In the responsible plaque group,there were 18 plaques with the significant enhancement,23 with mild enhancement,and 3 without enhancement.In the non-responsible plaque group,there were 10 plaques with the significant enhancement,17 with mild enhancement,and 13 without enhancement.The rate of plaque enhancement in the responsible plaque group was significantly higher than that of non-responsible plaque group(P<0.05).The degree of vascular stenosis,plaque enhancement index and plaque load in the responsible plaque group were significantly higher than those of the non-responsible plaque group,while the incidence of positive remodeling and remodeling index was significantly lower(P<0.05).After drug treatment,the degree of vascular stenosis,plaque enhancement index and plaque load in the responsible plaque group were significantly lower than those before treatment,while the incidence of positive remodeling and remodeling index was significantly higher(P<0.05).Conclusion HR-VWI can clearly localize and accurately analyze plaque characteristics of the responsible plaques of acute stroke,which can be used as an important evaluation method for the follow-up.After treatment,the responsible plaque load of acute stroke patients is reduced and plaque characteristics are improved.
