The correlation between Th17/Treg index and the occurrence of cerebral hemorrhage in patients with acute cerebral infarction after rt-PA thrombolysis therapy
Objective To analyze the correlation between helper T17(Th17)cells/regulatory t cell(Treg)index and the occurrence of cerebral hemorrhage after recombinant tissue plasminogen activator(rt-PA)thrombolysis in patients with acute cerebral infarction(ACI).Methods This study was a prospective analysis.The ACI patients admitted to the hospital from February 2021 to February 2023 were treated with rt-PA.The levels of Th17 and Treg cells were detected before and after thrombolytic therapy,and the levels of Th17/Treg were calculated.The patients were divided into a cerebral hemorrhage group and a non bleeding group according to the computed tomography(CT)/magnetic resonance imaging(MRI)results of thrombolysis treatment.The relationship between Th17/Treg and the occurrence of cerebral hemorrhage after rt-PA in ACI patients was tested by point biserial correlation.The receiver operating characteristic(ROC)curve was drawn to analyze the predictive value of Th17/Treg and the incidence of posttreatment cerebral hemorrhage.Results A total of 12/403(2.98%)ACI patients occurred cerebral hemorrhage after rt-PA thrombolytic were included.Before treatment,the levels of Th17/Treg of patients in the cerebral hemorrhage group were significantly higher than those in the non bleeding group(P<0.05).After treatment,the levels of Th17/Treg in both groups of patients decreased compared to before treatment,and the Th17/Treg levels in the cerebral hemorrhage group were significantly higher than those in the non bleeding group(P<0.05).Through point biserial correlation analysis,there was a positive correlation between Th17/Treg and the occurrence of cerebral hemorrhage in patients with acute cerebral infarction after rt-PA thrombolysis treatment(r=0.217,P<0.001).ROC curve showed Th17/Treg had certain predictive value for cerebral hemorrhage after rt-PA thrombolytic therapy in ACI patients(area under curve[AUC]= 0.809,P<0.001).Conclusion The Th17/Treg index is related to the occurrence of cerebral hemorrhage in ACI patients after rt-PA thrombolysis,and it can predict the occurrence of cerebral hemorrhage.
