Risk factors analysis and early warning model construction associated with high risk groups for cardiovascular disease of Kazak herdsmen in Nanshan pastoral area of Xinjiang

目的 分析新疆南山牧区哈萨克族牧民心血管病(CVD)高危人群危险因素并建立CVD预警模型,为CVD的预防控制提供参考依据.方法 2021 年 9 月至 2023 年 2 月采用多阶段分层整群随机抽样方法在新疆南山农牧区抽取 4 543 名哈萨克族牧民进行问卷调查和体格检查,分析CVD高危人群危险因素;多因素Logistic回归分析方法构建CVD预警模型,采用受试者工作特征曲线(ROC)对预警模型进行效果评价.结果 最终纳入分析的 4 543 名新疆南山牧区哈萨克族牧民CVD高危人群 1 428 名,CVD高危人群检出率为 31.4%;不同组别哈萨克族牧民各项指标比较,CVD高危人群BMI、WC、SBP、DBP、FPG、LDL-C、TG和TC均高于非CVD高危人群,差异均有统计学意义(P<0.05),HDL-C低于非CVD高危人群,差异有统计学意义(P<0.05);多因素非条件Logistic回归分析结果显示,CVD高危人群检出的危险因素包括年龄、文化程度、吸烟、饮酒、不进行锻炼、蔬菜水果摄入不达标、食盐量≥16.66 g/d、奶茶量≥1 000 mL/d、高脂饮食、食腌制熏肉、以动物油为主及鱼肉的较少摄入;纳入生活习惯(吸烟、饮酒、锻炼情况)、饮食习惯(蔬菜水果摄入情况、每日食盐量、奶茶量、高脂饮食、腌制熏肉摄入、用油及鱼肉摄入情况)和自测指标(BMI、SBP、DBP)进行多因素非条件Logistic回归分析,建立CVD高危预警模型为:Y=-20.163+0.275×吸烟+0.510×饮酒+0.433×未锻炼+0.211×蔬菜水果摄入未达标+0.246×食盐量≥16.66 g/d+0.241×奶茶过量+0.256×高脂饮食+ 0.302×食腌制熏肉+0.299×动物油为主+0.288×偶尔食鱼肉+0.51×不食鱼肉+0.042×BMI+0.029×SBP +0.145×DBP,预警模型ROC曲线下面积为0.868(95%CI=0.855~0.882),模型具有良好的预测效果.结论 新疆南山牧区哈萨克族牧民高危人群检出率相对较高,危险集中于生活、饮食、行为方面,构建的CVD高危预警模型预测效果较好.
Objective To analyze the risk factors of high risk groups for cardiovascular disease(CVD)and construct a early warning mode,aiming to provide reference for the prevention and control of CVD in Kazak herdsmen in Nanshan pastoral area of Xinjiang.Methods From September 2021 to February 2023,the questionnaire survey and physical examination of 4543 Kazak herdsmen in Nanshan pastoral area of Xinjiang were performed using multi-stage,stratified,cluster random sampling design.The risk factors of high risk groups for CVD were analyzed.CVD early warning model was constructed by Multivariate logistic regression analysis,and the receiver operating characteristic curve(ROC)was drawn to evaluate the effect.Results Totally 1,428/4,543(31.4%)cases of CVD high-risk groups of Kazak herdsmen in Nanshan pastoral area of Xinjiang were included in the analysis.BMI,waist circumference(WC),systolic blood pressure(SBP),diastolic blood pressure(DBP),fasting plasma glucose(FPG),low density lipoprotein cholesterol(LDL-C),triglyceride(TG)and total cholesterol(TC)in high-risk groups for CVD were significantly higher than those in high-risk groups for non-CVD,while HDL-C was significantly lower than that in high-risk groups for non-CVD(all P<0.05).Multivariate non-conditional logistic regression analysis showed that age,education level,smoking,drinking,no exercise,substandard intake of vegetables and fruits,daily salt≥16.66g,daily milk tea≥1 000mL,high-fat diet,intake of cured and smoked meat,mainly animal oils,and low intake of fish were risk factors for high risk groups for CVD.The lifestyle(smoking,drinking,exercise),dietary habits(intake of vegetables and fruits,daily salt and milk tea,high-fat diet,intake of cured and smoked meat,intake of oil and fish)and self-measured indicators(BMI,SBP,and DBP)were included for establishing a early warning model of CVD high-risk,the early warning model was as:Y=-20.163+0.275×smoking +0.510×drinking +0.433×no exercise +0.211×substandard intake of vegetables and fruits +0.246×salt intake≥16.66g/day+0.241×excessive milk tea +0.256×high fat diet +0.302×intake of cured and smoked meat +0.299×mainly animal oils +0.288×occasional fish consumption + 0.51×no fish consumption +0.042×BMI+0.029×SBP +0.145×DBP,the area under ROC curve of the early warning model was 0.868(95%CI:0.855-0.882),and the model had a good prediction effect.Conclusion The detection rate of high risk groups for CVD of Kazak herdsmen in Nanshan pastoral area of Xinjiang is relatively high.Lifestyle,diet and behavior are the main risk factors.The CVD high-risk early warning model has a good prediction effect.
