呼吸训练联合多形式健康教育改善COPD合并呼吸衰竭患者临床康复质量及心理状态研究

目的 探讨慢性阻塞性肺疾病(COPD)合并呼吸衰竭患者应用呼吸训练联合多形式健康教育对其临床康复质量及心理状态的影响.方法 选取 2021 年 3 月至 2022 年 3 月收治的COPD合并呼吸衰竭患者 100 例纳入研究,按随机数字表法分 2 组,每组 50 例.对照组采用常规护理,观察组在此基础上采用呼吸训练联合多形式健康教育进行干预,比较 2 组的肺功能、运动耐力、呼吸困难评分改善情况、疾病认知水平、生活质量和心理弹性水平.结果 干预后,2 组第 1 秒用力呼气容积(FEV1)、肺活量(FVC)和呼吸峰值流速(PEF)水平均升高,观察组高于对照组(P<0.05).干预后,2 组 6 min步行距离试验(6WMD)升高,观察组高于对照组,改良版呼吸困难量表(mMRC)评分降低,观察组低于对照组(P<0.05).观察组药物治疗、一般知识、个人防护等相关知识得分高于对照组(P<0.05).干预后,2 组圣乔治呼吸问卷(SGRQ)评分均降低,观察组低于对照组(P<0.05).干预后,2 组心理弹性评分均升高,观察组高于对照组(P<0.05).结论 COPD合并呼吸衰竭患者应用呼吸训练联合多形式健康教育,能有效改善患者的心理弹性水平,促进患者临床康复质量提高.
Objective To investigate the effect of breathing training combined with multiform health education on the clinical rehabilitation quality and psychological status of patients with chronic obstructive pulmonary disease(COPD)complicated with respiratory failure.Methods One hundred COPD patients with respiratory failure admitted to our hospital from March 2021 to March 2022 were recruited and randomly assigned into the observation group(n= 50)and the control group(n = 50).All patients were managed by routine nursing,and those in the observation group additionally received breathing training combined with multiform health education.The changes in pulmonary function,exercise endurance,dyspnea score,disease cognitive level,quality of life and psychological resilience of the two groups were included as comparators.Results After intervention,the patients in the two groups presented significantly higher forced expiratory volume in one second(FEV1),forced vital capacity(FVC)and peak expiratory flow(PEF),significantly increased the 6-min walk distance(6WMD),significantly decreased modified Medical Research Council(mMRC)dyspnoea score than those of before treatment(all P<0.05),and the improvements were more pronounced in the observation group than those of the control group(all P<0.05).After intervention,the scores of drug treatment,general knowledge,personal protection and other related knowledge in the observation group were significantly higher than those of the control group(P<0.05).The St.George's Respiratory Questionnaire(SGRQ)score and sychological resilience score were significantly higher than those of before treatment(both P<0.05),and the improvements were more pronounced in the observation group than those of the control group(both P<0.05).Conclusion For COPD patients with respiratory failure,breathing training combined with multiform health education can effectively improve the level of psychological resilience and promote the improvement of clinical rehabilitation quality.
