Construction and application of a special comprehensive management scheme for autologous arteriovenous fistulas in maintenance hemodialysis patients
Objective To construct a special comprehensive management scheme for autologous arteriovenous fistulas in maintenance hemodialysis patients and to analyze its application outcome.Methods A total of 90 maintenance hemodialysis patients undergoing autologous arteriovenous fistulas in our hospital from January 2021 to June 2022 were included as study objects.They were randomly divided into the observation group and control group,with 45 cases in each group.All patients were intervened with a routine management of autologous arteriovenous fistulas,and those in the observation group were additionally intervened by the special comprehensive management of autologous arteriovenous fistulas.Internal fistula puncture and use,internal fistula maturity,dialysis adequacy,internal fistula nursing ability and nursing satisfaction were compared between the two groups.Results The fistula diameter and blood flow at 4 weeks and 8 weeks after surgery were significantly larger in the observation group than those of the control group,and the vascular depth was significantly lower(P<0.05).The maturation time of internal fistulas in the observation group was significantly shorter than that of control group,and the success rate of one-time puncture,single-pool urea Kt/V(spKt/V)and blood flow at the first use of a dialysis pump were significantly higher than those of control group(P<0.05).The score of the internal fistula self-management scale in the observation group was significantly higher than that of control group(P<0.05).The complication rate of observation group was significantly lower than that of control group(P<0.05).The nursing service satisfaction scores of observation group were significantly higher than those of control group(P<0.05).Conclusion The construction of a special comprehensive management scheme in patients with autologous arteriovenous fistula in maintenance hemodialysis is helpful to improve the self-management ability,maturity of internal fistula,quality of internal fistula and the adequacy of dialysis.and reduce the risk of complications of internal fistulas.
