Analysis of the diagnostic value of serum miR-30a-3p combined with Helicobacter pylori antibody detection for early gastric cancer in the elderly
Objective To explore the diagnostic value of serum microRNA-30a-3p(miR-30a-3p)combined with Helicobacter pylori(H.pylori)antibody detection for early gastric cancer in the elderly.Methods From May 2020 to December 2022,87 elderly patients with gastric diseases who underwent gastroscopy in our hospital were collected as the study subjects.According to the pathological examination results,patients were grouped into chronic atrophic gastritis(CAG)group(n=39),early gastric cancer group(n=27)and advanced gastric cancer group(n=21).During the same period,33 healthy individuals were regarded as the control group.Fluorescence quantitative PCR(qRT-PCR)and chemiluminescence were applied to detect serum miR-30a-3p expression level and H.pylori antibodies,respectively.Serum miR-30a-3p and the positive rate of H.pylori antibodies were compared.Serum miR-30a-3p level was compared between patients with positive versus negative H.pylori antibody.Logistic regression was applied to analyze the risk factors for early gastric cancer in the elderly.Receiver operating characteristic(ROC)curves were plotted to analyze the diagnostic value of serum miR-30a-3p and H.pylori antibodies for early gastric cancer in the elderly.Results The serum miR-30a-3p level in the early gastric cancer group and advanced gastric cancer group was significantly lower than that in the CAG group and control group(P<0.05).The positive rate of H.pylori antibody in the early gastric cancer group,advanced gastric cancer group,and CAG group was significantly higher than that in the control group(P<0.05).Among the 87 patients with gastric diseases and 33 healthy individuals who underwent physical examination,serum miR-30a-3p level in individuals with positive H.pylori antibody was significantly lower than that in those with negative H.pylori antibody(P<0.05).Logistic regression analysis showed that serum miR-30a-3p and positive H.pylori antibodies were risk factors for early gastric cancer in the elderly(P<0.05).The area under the curve(AUC)of serum miR-30a-3p combined with H.pylori antibody in the diagnosis of early gastric cancer in the elderly was 0.957,which was larger than that of a single detection of serum miR-30a-3p or H.pylori antibody(Zcombination-miR-30a-3P=2.680,Zcombination-H.pylori antibody=4.577,P=0.007,P<0.001).The sensitivity and specificity of combined examination of serum miR-30a-3p and H.pylori antibody were 85.19%and 96.67%,respectively.Conclusion Serum miR-30a-3p is lowly expressed in elderly patients with early gastric cancer,and the positive rate of H.pylori antibody is high.Their combined detection has a high diagnostic value for early gastric cancer in the elderly.
