Early application of cross joint external fixators for protecting the soft tissue of the knee joint in patients with tibial plateau fractures
Objective To explore the protective effect of early application of cross joint external fixators on knee joint soft tissue in patients with type Ⅵ tibial plateau fractures. Methods A total of 122 patients with type Ⅵ tibial plateau fractures admitted to our hospital from January 2019 to December 2020 were selected as the study subjects. They were randomly assigned into the study group(n=61)and the control group(n=61)using the envelope method.Patients in the study group were treated with early application of cross joint external fixators within 12 hours of admission combined with internal fixation surgery.Patients in the control group patients were treated with delayed application of cross joint external fixators within 24-72 hours of admission combined with internal fixation surgery. The surgical performance indicators, postoperative recovery indicators, knee joint efficacy, knee joint function scores, and knee joint soft tissue injury indicators between the two groups were observed and compared. Results The external and internal fixation surgery time in the study group was significantly shorter than that of the control group(P<0.05).Patients in the study group had significantly lower Visual Analogue Scale(VAS)score after external fixation surgery and less bleeding during internal fixation surgery than those of the control group(P<0.05).The rehabilitation time and fracture healing time of the study group were significantly shorter than those of the control group(P<0.05).There were no significant differences in the incidence of complications and the excellent rate of knee treatment between the two groups(P>0.05).After surgery, the knee joint function scores of both groups decreased, which were significantly lower in the study group than those of the control group(P<0.05).Patients in the study group had significantly lower incidence of blisters after external fixation surgery, lower degree of swelling after external fixation surgery and faster healing of blister scabs than those of control group(P<0.05).Conclusion Compared with the delayed application, early treatment of type Ⅵ tibial plateau fractures with cross joint external fixators and internal fixation surgery can shorten the surgical time, improve fracture healing and postoperative rehabilitation progress, and promote the recovery of knee joint function and soft tissue injuries.
