Preliminary study on the relationship between atherosclerotic ischemic stroke and intracerebral hemorrhage
Stroke is a major disease that seriously affects human health.Stroke is usually classified into ischemic stroke(IS)and intracerebral hemorrhage(ICH).Diagnosis and treatment guidelines for the management of IS and ICH vary according to their pathological characteristics. However,a simultaneous onset of both IS and ICH can be observed in clinical practice, which greatly challenge the treatment.Through literature review of the etiology, pathology, imaging features, and clinical cases of atherosclerotic stroke, this review aims to explore the basic pathological changes and process of IS combined with ICH or ICH combined with IS in atherosclerotic stroke, thus illustrating the nature and correlation of ICH and IS.The analysis showed that atherosclerotic stroke, whether IS or ICH,has the common etiology, pathology, and imaging characteristics.Arterial wall thickening, unstable plaque formation, and lumen stenosis lead to IS,while small artery wall sclerosis and increased fragility lead to ICH.These two vascular lesions often coexist in one patient.ICH and IS are two manifestations of atherosclerotic stroke, serving as the unity of opposites.A comprehensive consideration of the vascular pathological basis in both IS or ICH is essential in the diagnosis and treatment of IS or ICH.An individualized treatment to strictly control and management the co-existed risk factors for IS and ICH favors better prevention and treatment of atherosclerotic stroke.
