Objective To comprehensively review the outpatient prescriptions of sample patients in our hospital,thus proposing intervention measures.Methods A total of 5460 outpatient review prescriptions from October to December 2022 were randomly collected and reviewed one by one according to the regulations.The proportion of irrational prescriptions and the key problems were summarized.Results Among reviews for 5460 outpatient prescription collected in this study,there were 368(6.7％)irrational drug use,which mainly included non-standard writing(12.8％),abnormal prescription(14.7％),off-label drug use(19.0％),adjuvant drug use(17.1％)and improper usage and dosage(10.6％).Conclusion Our hospital will strictly implement the principle of pharmacist prescription review,continuously strengthen the prescription review process,implement pre prescription real-time review,promote rational medication,ensure patient medication safety,and improve clinical medical quality.
