Correlation of serum miR-340-5p and Bmi-1 with vascular cognitive impairment in elderly patients with acute cerebral infarction
Objective To investigate the correlation of serum microRNA-340-5p(miR-340-5p)and B-lymphoma moloney murine leukemia virus insertion region-1(Bmi-1)with vascular cognitive impairment(VCI)in elderly patients with acute cerebral infarction(ACI).Methods From March 2021 to February 2023,103 elderly patients with ACI were collected as the study subjects.According to the Montreal Cognitive Assessment(MoCA)score,patients were divided into VCI group(n=60)and non-VCI group(n=43).The general information in two groups was compared.Real-time fluorescence quantitative PCR(RT-qPCR)was applied to detect serum expressions of miR-340-5p and Bmi-1.Using the ENCORI database,the targeting relationship between miR-340-5p and Bmi-1 was predicted.Pearson correlation was applied to analyze the correlation between serum miR-340-5p and Bmi-1 levels in VCI patients.Spearman correlation was applied to analyze the correlation of serum levels of miR-340-5p and Bmi-1 with the MoCA score.Logistic regression was applied to analyze the influencing factors of VCI in elderly patients with ACI.Receiver operating characteristic(ROC)curves were plotted to analyze the diagnostic value of serum miR-340-5p and Bmi-1 levels for the occurrence of VCI in elderly patients with ACI.Results The proportions of patients with history of smoking and previous cerebral infarction in the VCI group were significantly higher than those of the non-VCI group(P<0.05),and the MoCA score was significantly lower(P<0.05).Compared with those of the non-VCI group,significantly downregulated miR-340-5p and upregulated Bmi-1 were detected in the VCI group(P<0.05).The prediction results of the ENCORI revealed a targeting relationship between miR-340-5p and Bmi-1.In VCI group,serum miR-340-5p was negatively correlated with serum Bmi-1,but positively correlated with the MoCA score(P<0.05).Serum Bmi-1 was negatively correlated with the MoCA score in VCI patients(P<0.05).Logistic regression analysis showed that MoCA score and miR-340-5p were protective factors affecting the occurrence of VCI in elderly patients with ACI(P<0.05),while smoking history,history of previous cerebral infarction,and Bmi-1 were risk factors affecting the occurrence of VCI in elderly patients with ACI(P<0.05).ROC curve analysis showed that serum levels of miR-340-5p(area under the curve[AUC]=0.851)and Bmi-1(AUC=0.886)had a good diagnostic potential of VCI in elderly patients with ACI,and their combination had a better diagnostic effect(AUC=0.947,Z combination-miR-340-5p=3.018,P=0.003;Z combination-Bmi-1=2.636,P=0.008).Conclusion MiR-340-5p is downregulated and Bmi-1 is upregulated in the serum of elderly patients with VCI after ACI.The expression levels of both are closely related to the occurrence of VCI in elderly patients with ACI.
