Diabetic retinopathy(DR)is one of the most common chronic complications of diabetes mellitus(DM)and is the leading cause of blindness among working-age populations.Its fundus manifestations include retinal microaneurysms,hemorrhage,ede-ma,exudation,vitreous hemorrhage,and even retinal detachment,which fall into the categories of xiao ke mou bing(diabetes-in-duced ophthalmic disease),shi zhan hun miao(blurred vision),and yun wu yi jing(floating cloud-like vision)in traditional Chinese ophthalmology.The fundus lesions progressively worsen,and the etiology and pathogenesis are complex,involving both deficiency and excess.Based on the theory of ocular collaterals and years of clinical research,Professor Zhang Minglian believes that the main patho-logical mechanism of DR is "obstruction of ocular collaterals due to stagnation(or blood stasis)",and the Tiaoxue Tongluo Therapy(Therapy of regulating blood circulation and unblocking collaterals)is the primary treatment approach.This article will elaborate on the understanding and clinical application of Tiaoxue Tongluo Therapy in the treatment of DR based on the theory of ocular collaterals.
