Relationship between Peripheral Blood NLR,PLR and Prognosis of Newly Diagnosed Multiple Myeloma Patients
Objective To investigate the relationship between peripheral blood neutrophil to lymphocyte count ratio(NLR)and platelet to lymphocyte count ratio(PLR)and prognosis of newly diagnosed multiple myeloma(MM)patients.Methods The general data of 86 newly diagnosed MM patients wereretrospectively analyzed.General data(gender,age,ISS stage)of the patients were collected,and the levels of granulocyte count,lymphocyte count,platelet count,Hb,ALB,Cr,Ca,β2 microglobulin were detected,and NLR and PLR were calculated.According to the levels of NLR and PLR,patients were divided into high NLR group(NLR≥2.54,n=32),low NLR group(NLR<2.54,n=54),high PLR group(PLR≥135,n=41)and low PLR group(PLR<135,n=45).Kaplan-Meier curve was used to determine the overall survival time(OS)of patients,and Log-Rank test was performed to analyze the survival of patients with different NLR and PLR levels.A risk assessment model was established,and the diagnostic efficacy of NLR and PLR in evaluating the prognosis of newly diagnosed MM patients was analyzed by receiver operating characteristic curve(ROC).Results The levels of ISS staging,Ca,Cr and β2 microglobulin between high NLR group and low NLR groupwere significant differences(P<0.05).There were significant differences in ISS stage,Cr and β2 microglobulin levels between high PLR group and low PLR group(P<0.05).The survival time of high NLR group was(13.28±1.99)months,and that of low NLR group was(36.15±5.42)months.The survival time of high NLR group was shorter than that of low NLR group(χ2=13.984,P=0.000<0.05).The survival time of high PLR group was(25.31±3.79)months,and that of low PLR group was(33.26±4.98)months.The survival time of high PLR group was shorter than that of low PLR group(χ2=8.767,P=0.003<0.05).Based on the ROC curve analysis the AUC value of NLR and PLR combined detection mode was the high-est,which was 0.923,and the sensitivity was 92.85%、the specificity was 85.71%.Conclusion The levels of NLR and PLR in peripheral blood are closely related to the prognosis of newly diagnosed MM patients.The levels of NLR and PLR in patients with MM were negatively correlated with the prognosis and survival time.At the same time,the combined detection of NLR and PLR has high clinical value in evaluating the prognosis of newly diagnosed MM patients.
