Expression of serum sIL-2R,PGRN and Copeptin in children with severe pneumonia and their correlation with prognosis
Objective:To investigate the expression of serum soluble interleukin-2 receptor(sIL-2R),serum granuloprotein precur-sor(PGRN),and peptide(Copeptin)in children with severe pneumonia,and to analyze their correlation with prognosis.Methods:A total of 183 children with pneumonia admitted to our hospital from January 2021 to December 2022 were selected as the study objects.According to the severity of the disease,the patients were divided into a severe pneumonia group(n =80)and a common pneumonia group(n =103),and another 100 healthy children were selected as the healthy group.Serum levels of sIL-2R,PGRN and Copeptin were compared between the three groups at admission,and the diagnostic value of each index was analyzed by ROC.The levels of ser-um indicators,critical case score(PCIS)and acute physiological chronic health score(APACHEII)of children with different progno-sis were compared at admission,and the risk of poor prognosis in children with different levels of serum indicators at admission was an-alyzed.Results:The levels of serum sIL-2R,PGRN,and Copeptin in the severe pneumonia group(648.47±100.27 U/mL,36.17±10.47 μg/L,62.77±14.21 pmol/L)were higher than those in the common pneumonia group(461.51±79.64U/mL,30.66±7.21μg/L,47.66±11.47pmol/L)and those in the healthy group(214.79±39.47 U/mL,26.31±4.14 μg/L,11.27±6.94 pmol/L)(P<0.05).The AUC of serum sIL-2R,PGRN,and Copeptin levels at admission in combined detection for the diagnosis of severe pneumonia was 0.789.The serum sIL-2R,PGRN,Copeptin,and APACHEII scores of children with poor prognosis were(715.98±79.51 U/mL,43.79±8.97 μg/L,68.49±6.79 pmol/L,22.81±3.79),which were higher than those of children with good prog-nosis(627.44±45.69 U/mL,33.82±8.46 μg/L,60.99±7.21 pmol/L,20.16±3.68).The PCIS score of children with poor prognosis(72.14±6.44 scores)was lower than that of children with good prognosis(81.51±7.41),with significant differences(P<0.05).Serum sIL-2R,PGRN and Copeptin were negatively correlated with PCIS score and positively correlated with APACHEII score(P<0.05).The risk of poor prognosis in children with high levels of serum sIL-2R,PGRN and Copeptin at admission was 2.981,4.637,and 3.458 times of that with low levels(P<0.05).Conclusion:Serum sIL-2R,PGRN and Copeptin are significantly expressed in children with severe pneumonia,which is closely related to the prognosis of severe pneumonia,and can be used as auxiliary indicators to predict the prognosis of children with severe pneumonia.
