Expression of serum FGF19 and FGF21 in gestational diabetes patients with different pre-pregnancy body mass index and their correlation with insulin resistance
Objective:To investigate the correlation between serum levels of fibroblast growth factor 19(FGF19)and fibroblast growth factor 21(FGF21)and insulin resistance in pregnant women with different pre-pregnant body mass index.Methods:A total of 120 pregnant women who underwent oral glucose tolerance test(OGTT)in a hospital from June 2019 to June 2022 were selected as the study objects.Based on the results of oral glucose tolerance test,they were divided into a NGT group(45 cases,OGTT results negative for pregnant women as normal control group)and a GDM group(75 cases,gestational diabetes mellitus).According to pre-pregnancy body mass index(pre-pregnancy BMI),The GDM group was re-divided into a normal weight group(GDM1 group,37 cases,pre-pregnancy BMI<24Kg/m2)and a super-recombination group(GDM2 group,38 cases,pre-pregnancy BMI≥24 Kg/m2).General clinical biochemical indexes were determined such as TG,TC,LDL-C,HDL-C,FGF19 and FGF21.SPSS 26.0 software was used for statistical analysis of the data.Results:The results of variance analysis or rank sum test showed no statistical significance in age,gesta-tional age,HOMA-β,TC and LDL-C levels among the three groups(P>0.05),while statistically significant differences were mani-fested in pre-pregnancy BMI,FPG,FINS,HOMA-IR,TG and HDL-C(P<0.05).SerumFGF19 levels were statistically different a-mong the 3 groups,with GDM2 group<GDM1 group<NGT group[(11.32±4.61)<(15.55±2.87)<(21.87±2.98),P<0.05].Serum FGF21 level in GDM2 group was higher than that in NGT group and GDM1 group.Serum FGF21 level in GDM1 group was not significantly different from that in NGT group[(40.62±3.12)VS(29.55±3.26)/(27.60±2.35),P<0.05].The corre-lation analysis showed that the FGF19 level in GDM group was negatively correlated with HOMA-IR and the pre-pregnancy BMI(r=-0.353,-0.484,P<0.05),while FGF21 level was positively correlated with HOMA-IR and the pre-pregnancy BMI(r=0.251,0.418,P<0.05).The results of multivariate regression analysis showed that pre-pregnancy BMI was the main influencing factor of FGF19 and FGF21,and FPG was also the influencing factor of FGF19.FGF19 and FGF21 were mutually influencing factors.Multivari-ate Logistic regression model showed that pre-pregnancy BMI and FGF19 had statistically significant effects on GDM.Conclusion:The levels of FGF19 and FGF21 in the serum of gestational diabetes patients with different pre-pregnancy body mass index were different,which were correlated with insulin resistance.FGF19 and FGF21 may delay the progression of GDM.
