Analysis of influencing factors of short-term prognosis in young patients with first-time ischemic stroke
Objective:To explore the factors that influence prognosis of first-time young ischemic stroke(IS)patients 7 days after hospitalization and 90 days after discharge.Methods:General data of 81 patients with IS who were hospitalized in Bengbu Third People's Hospital from February 2022 to September 2023 were collected.According to the modified RANKIN scale(mRS)score at 7 days after hospitalization,the patients were divided into a poor prognosis group(mRS≥2 points)(18 cases)and a good prognosis group(mRS<2 points)(63 cases).Based on the mRS score at 90 days after discharge,the patients were divided into a poor prognosis group(mRS≥2 points)(12 cases)and a good prognosis group(mRS<2 points)(69 cases).Univariate analysis was used to com-pare the risk factors between the poor prognosis group and the good prognosis group,and multi-factor Logistic regression was used to an-alyze the independent risk factors affecting the poor prognosis at 7 days in hospital and 90 days after discharge.Results:The results of single factor analysis showed that in the 7d hospitalization group,National Institutes of Health Stroke Scale(NIHSS),rate of type 2 di-abetes,total cholesterol,triglyceride,lowdensity lipoprotein(LDL),homocysteine(Hcy)and TyG indexes were higher than those in the good prognosis group(P<0.05).At 90 days after discharge,NIHSS,rate of type 2 diabetes,total cholesterol,Hcy,and TyG inde-xes in the poor prognosis group were higher than those in the good prognosis group(all P<0.05).Logistic regression analysis showed that NIHSS(OR=2.056,95%CI:1.010~4.187)and TyG index(OR=2.139,95%CI:1.039~4.402)were independent risk fac-tors for poor prognosis on the 7th day after hospitalization(P<0.05).NIHSS(OR=2.949,95%CI:1.141~7.620),TyG index(OR=5.026,95%CI:1.047~5.088)and Hcy(OR=2.075,95%CI:1.023~4.210)were independent risk factors for poor prognosis at 90 days after discharge(P<0.05).Conclusion:High TyG index and high NIHSS on admission were the independent risk factors for the prognosis of young first-time IS patients at 7 days after hospitalization,while high TyG index,high NIHSS on admission and high blood Hcy were the independent risk factors for the prognosis at 90 days after discharge.
