Influencing factors and prognosis of early exacerbation of neurological impairment in patients with progressive cerebral in-farction
Objective:To explore the influential factors and prognosis of early exacerbation of neurological impairment(END)in patients with progressive cerebral infarction.Methods:Clinical data of 150 patients with progressive cerebral infarction admitted from January 2019 to June 2023 were included,and the patients were divided into an END group(58 cases)and a non-END group(92 ca-ses).The baseline data,biochemical indexes,imaging examination,neurological function and other data of the END group and the non-END group were compared and analyzed.According to the modified Rankin Scale(mRs)score,the patients were divided into a good prognosis group(mRS Score<3,88 cases)and a poor prognosis group(mRS Score≥3,62 cases),and the independent factors affecting the END of patients with progressive cerebral infarction and the related factors 30 days after the onset of the disease were ana-lyzed.Results:The hypertension history,NIHSS,D-D and carrier artery stenosis>50%levels in the END group were higher than those in the non-END group(P<0.05).The hypertension history,carrier artery stenosis>50%,lesion diameter and END rate after admis-sion in the poor prognosis group were higher than those in the good prognosis group(P<0.05).Multivariate logistic analysis showed that history of hypertension,NIHSS at admission,D-D and carrier artery stenosis>50%were independent risk factors for END in pa-tients with progressive cerebral infarction.History of hypertension,carrier artery stenosis>50%and concurrent END were independent risk factors for adverse prognosis in patients with progressive cerebral infarction.Conclusion:Hypertension,NIHSS score at admission,D-D and carrier artery stenosis are the influential factors of progressive cerebral infarction complicated with END.It is important to take timely intervention measures to improve the clinical symptoms and prognosis of progressive cerebral infarction.
